About The Role

The Clinical Administrative Coordinator is a vital remote position within our Care Transition product team, primarily responsible for serving as the main point of contact for healthcare providers and members receiving post-acute services. This role involves providing administrative support to our multidisciplinary clinical team, ensuring smooth communication and coordination during patient transitions from hospital to home. The Coordinator plays a key role in managing inbound and outbound communications, maintaining accurate documentation, and supporting the overall continuum of care. This full-time position requires flexibility to work Saturday through Tuesday, with shifts scheduled during normal business hours of 7:00 am to 5:30 pm CST. The role offers comprehensive training and opportunities for professional growth within a dynamic, innovative healthcare environment.

Qualifications

  • High School Diploma / GED or equivalent work experience
  • Must be 18 years of age or older
  • Minimum 1+ years of experience in a call center environment
  • Proficiency in communication via telephone, email, and messaging platforms such as Microsoft Teams
  • Ability to work a 10-hour shift schedule Saturday - Tuesday from 7:00 am to 5:30 pm CST
  • Flexibility to work occasional overtime or weekends as required

Preferred Qualifications

  • Familiarity with medical terminology and HIPAA laws
  • Experience working in a healthcare setting
  • Background in high-volume, production-based environments
  • Proven track record of meeting performance goals
  • Excellent written and verbal communication skills
  • Critical thinking and problem-solving abilities
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams)
  • Strong attention to detail and data interpretation skills
  • Ability to prioritize and organize multiple tasks effectively

Responsibilities

  • Manage inbound and outbound telephone communications professionally, adhering to productivity and quality standards.
  • Process electronic data and communication, including faxes, emails, and system notifications, efficiently and accurately.
  • Enter, review, and verify member information within the Home & Community Care documentation system, ensuring data integrity.
  • Maintain and update member records, including disenrollment, care transitions, authorization creation, and documentation attachments.
  • Coordinate member care between healthcare providers and the health plan to facilitate seamless transitions and continuity of care.
  • Establish and maintain effective communication with providers, clients, and care management staff, respecting departmental policies.
  • Triage and respond to inbound calls and emails, resolving inquiries or escalating issues to appropriate teams.
  • Generate and review reports to monitor workflow, performance metrics, and task completion.
  • Serve as a mentor to new team members, providing training and support during onboarding.
  • Support operational and procedural questions, acting as a first-line peer resource.
  • Follow established chain of command for issue resolution and escalation.
  • Participate in team reviews, resource allocation, and process improvement initiatives.
  • Create and update training materials and conduct quality audits as needed.
  • Assist with special projects, sales tickets, and time studies to support team efficiency.
  • Attend departmental meetings and contribute to team engagement activities.
  • Adhere to organizational policies, compliance standards, and regulatory requirements.
  • Perform additional duties as assigned to support team success and organizational goals.
