Organization Summary Harvest Home is a Christian nonprofit organization that equips and empowers unhoused pregnant women and their children through housing, support, and holistic programming designed to strengthen families and create pathways toward long-term stability. Harvest Home serves women through a continuum of care model that includes residential services, family stabilization support, emotional wellness services, alumni engagement, and community-based programming rooted in dignity, compassion, accountability, and hope. Position Summary The Alumnae Therapeutic Care Manager serves as the primary coordinator of Harvest Home's Alumnae Program. This role supports mothers and families as they transition from residential services into long-term community stability by providing therapeutic case management, attachment-centered support, parenting education, wellness programming, and alumni engagement services. Working from a trauma-informed, relationship-centered, and strengths-based framework, the Alumnae Therapeutic Care Manager walks alongside participants as they navigate the challenges of parenting, employment, education, housing stability, emotional wellness, and community integration. The position recognizes that practical stability and emotional wellness are deeply interconnected and seeks to support both through individualized services, group programming, and ongoing community connection. The Alumnae Therapeutic Care Manager serves as both a therapeutic support and a bridge between participants and the broader community resources necessary for long-term success. Through therapeutic case management, dyadic parent-child interventions, alumni engagement, and wellness programming, this position helps families strengthen resilience, increase self-sufficiency, and build healthy foundations for themselves and their children. Essential Duties & Responsibilities Alumnae & Family Stabilization Services Provide therapeutic case management services to alumni participants, Family Program participants, and other assigned families using a relationship-based and trauma-informed approach that supports both emotional wellness and practical stabilization. Services include ongoing assessment, goal development, resource coordination, emotional support, crisis intervention, transition planning, and individualized coaching related to housing stability, employment, education, childcare, transportation, healthcare access, financial wellness, and long-term self-sufficiency. Utilize Critical Time Intervention (CTI) principles to support participants during periods of transition, increased vulnerability, or instability. Assist participants in strengthening support networks, increasing community connections, developing sustainable routines, and reducing the likelihood of future crises. Maintain ongoing therapeutic relationships with participants that promote emotional regulation, resilience, self-advocacy, healthy decision-making, and progress toward individualized goals while maintaining a strong focus on participant strengths and long-term independence. Provide short-term stabilization and re-engagement services for alumni who may be experiencing housing instability, parenting stress, emotional distress, family challenges, or other barriers that place their long-term stability at risk. Attachment, Parenting & Wellness Programming Provide attachment-centered and dyadic interventions that strengthen parent-child relationships, increase caregiver confidence, and promote healthy social-emotional development. Through observation, coaching, modeling, and reflective conversations, support caregivers in understanding their children's developmental needs, emotional communication, attachment cues, and regulation patterns. Facilitate parent-child sessions, attachment-based play groups, developmental coaching opportunities, and family wellness activities designed to strengthen co-regulation, attunement, responsiveness, and emotional safety within the parent-child relationship. Provide education and support related to developmental milestones, age-appropriate expectations, infant and early childhood mental health, emotional regulation, sensory needs, executive functioning development, and healthy family boundaries. Particular attention should be given to helping families recognize and interrupt patterns of parentification, role reversal, and unrealistic developmental expectations. Coordinate and facilitate parenting classes, psychoeducational workshops, and family enrichment activities that promote healthy child development, responsive caregiving, emotional wellness, and family resilience. Facilitate wellness-based programming that supports participant self-care and nervous system regulation, including yoga, somatic regulation activities, guided breath work, mindfulness practices, movement-based wellness opportunities, and discussions related to nutrition, sleep, stress management, and overall well-being. Support participants in accessing resources that promote food security and healthy living, including fresh fruits and vegetables, community nutrition resources, wellness education, and practical strategies for incorporating healthy habits into daily family life. Alumnae Engagement, Community Partnerships & Program Development Serve as the primary point of contact for Harvest Home alumnae and work to foster meaningful, ongoing engagement beyond residential services. Build relationships with alumnae through regular outreach, support, encouragement, and connection to services that strengthen long-term family stability. Coordinate and support alumnae-focused events, support groups, workshops, family activities, volunteer opportunities, and community-building experiences that strengthen participants' sense of belonging, connection, and ongoing engagement with Harvest Home. Develop and maintain collaborative relationships with community providers, service partners, educational institutions, childcare providers, healthcare organizations, faith communities, and wellness organizations that expand opportunities and resources available to participants and alumnae . Assist in the continued development, implementation, and evaluation of Harvest Home's Therapeutic CTI and Family Wellness Program. Participate in quality improvement efforts, participant feedback collection, outcome tracking, and program planning to ensure services remain responsive to participant needs and aligned with organizational goals. Collaborate with leadership to identify opportunities for growth, innovation, and enhancement within the alumni continuum of care and family wellness programming. Spiritual Formation & Community Support Create a spiritually supportive environment that is welcoming, emotionally safe, trauma-informed, and grounded in grace, compassion, and respect for each participant's unique faith journey. Documentation, Collaboration & Organizational Participation Maintain accurate , timely , and professional documentation of all participant services, interventions, referrals, contacts, progress notes, assessments, and outcomes in Apricot and other organizational systems. Participate in multidisciplinary meetings, reflective supervision, case conferencing, staff trainings , program planning meetings, and organizational initiatives to ensure coordinated and effective service delivery. Collaborate closely with Program, Residential Life, Emotional Wellness, Volunteer, and Development staff to ensure participants receive comprehensive support and services that align with Harvest Home's mission and values. Assist with organizational events, community events, fundraising activities, participant celebrations, and other agency initiatives as needed. Schedule This position is generally scheduled Monday through Friday during regular business hours. The role includes occasional evening responsibilities and monthly weekend commitments to support participant programming, alumni events, community activities, and organizational events. Harvest Home supports flexible scheduling when appropriate to meet participant needs and organizational priorities. Hybrid work opportunities may be available, including the ability to work remotely at least one day per week, with the potential for additional remote work flexibility based on program needs and performance. Qualifications Master's degree in Marriage and Family Therapy, Clinical Psychology, Social Work, Counseling, or a related behavioral health field. Associate registration or clinical licensure requi red . Experience working with pregnant and parenting families, maternal mental health, infant and early childhood mental health, family stabilization, homelessness or housing instability, community mental health, and trauma-informed care. Strong understanding of attachment theory, co-regulation, child development, developmental milestones, family systems, parentification, reflective practice, and strengths -based interventions. Experience facilitating groups, providing case management services, and supporting diverse populations from a culturally responsive and trauma-informed perspective. Excellent communication, organization, documentation, and relationship-building skills. Ability to work collaboratively within a multidisciplinary team while maintaining professional boundaries and a participant-centered approach.
