Join AmeriCorps as a Social Determinants of Health (SDOH) Health Educator at Erie Family Health Centers. This year-long, full-time service position provides the opportunity to help connect individuals and families to resources that support their health and well-being. This is a hands-on experience in community outreach, resource navigation, benefits enrollment, and patient support while building professional skills through training and service. The work schedule for this position is aligned with Erie's hours of operations. Main Duties & Responsibilities Screen individuals and families for social drivers of health needs and connect them with appropriate resources. Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs. Assist with enrollment in eligible benefits, programs, and services. Support outreach activities and follow-up with individuals regarding resource needs. Provide digital navigation assistance and help individuals access available services. Complete documentation, data entry, and program tracking activities. **Applications must be submitted by September 13, 2026, to be considered for full-time opportunities. Anticipated start of Monday, September 21st 2026. Qualifications Education Required : High school diploma or equivalent. Skills and Knowledge Required: Strong verbal and written communication skills. Current CPR Certification. Ability to work with diverse populations. U.S. citizen, national, or lawful permanent resident. Ability to meet AmeriCorps background check requirements. Preferred: Experience in community health, social services, or related fields. Intermediate Microsoft Office skills. Experience with electronic health record systems. Bilingual English/Spanish skills.
