Job Summary In this role, the Behavioral Health Counselor will primarily support patients referred from the Saugus Internal Medicine practice, while also providing virtual coverage to the Lynn Primary Care practice as needed. Working as part of an integrated care team, the counselor will collaborate closely with primary care providers, nurses, and social work case management. Responsibilities include conducting behavioral health assessments, developing treatment plans, and providing brief interventions. Patient appointments typically range from 30-60 minutes, with an average of 6-8 patients per day. The counselor will work independently while also being supported by other licensed clinicians (LICSW) across the team. We are seeking a Full-time 40-hour Social Worker to support our office Monday through Friday from 8:30am - 5:00pm. As a hybrid opportunity, the position entails onsite coverage 3 days per week on site at our Internal Medicine office located at 1069 Broadway Street in Saugus and remote coverage 2 day per week. Our social worker will work within the Integrated Behavioral Health Service; working closely with primary care and other specialty services to deliver exceptional care to our patients across the age spectrum. We frequently prototype new ideas, including digital tools, care team roles, and workflows, that may be used in designing for the future of ambulatory care across MGB Integrated Care. Responsibilities to include: -Providing short-term, evidence-based psychotherapy (individual, families and groups) to patients of the primary care practice in-person and virtually -Developing comprehensive assessments and treatment plans for patients engaged in BH treatment -Providing a range of interventions to support primary care as needed such as crisis intervention; care coordination; information; referrals; and safety planning. -Assessing new patients to aid in the identification of the best treatment options for them -Participating in formal and informal case reviews, seminars and / or inter- and intra-departmental peer review meetings -Supporting patients in accessing resources and managing care -Working closely with the other members from the Behavioral Health Team as well as the Primary Care and Collaborative Care Teams to coordinate services needed to improve the lives of our patients. Qualifications Education Master's Degree Related Field of Study required License LMHC or LICSW in MA Experience At least 3 years of related experience required.
