Job Summary: Provides comprehensive assessment and disposition of mental, emotional, behavioral, addictive, and developmental disorders/disabilities using specialized clinical knowledge and advanced clinical skills in a virtual environment. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Provides comprehensive biopsychosocial and standardized assessments using specialized clinical knowledge and advanced clinical skills for potential program enrollment. Works with the patient to develop an initial individualized support plan with goals. Provides resources for services in the community and makes referrals. Provides enrollment and intake services to patients and potential patients. Responds to telephone inquiries from persons seeking mental health care or information about mental health related services. Conducts safety assessment and planning with patients via telephone or video. Partners with patients to create self-directed sustainable changes that align with their values to promote health/wellness and enhance well-being. Collaborates with other members of the multidisciplinary team to develop support goals for the patient. May participate in multidisciplinary patient care conferences discussing information obtained from patient assessments, updates, and plans. Provides appropriate documentation of all pertinent information obtained in interviews, consultation with other healthcare professionals, referrals, contacts, and supportive services provided to each patient. Completes necessary documentation and reporting in accordance with departmental standards and federal and state regulations. Works in a constant state of alertness and safe manner. Performs other duties as assigned.
