You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times). Key Responsibilities Conduct home visits to assess and create personalized care plans. Train and support caregivers, ensuring they can meet their loved ones' needs. Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more. Facilitate smooth transitions and ongoing care for clients. Maintain compliance with all relevant regulations and standards. Full-time hybrid position offering autonomy while utilizing your assessment, care planning and case management skills. If you're ready to make a difference in the lives of others, apply today! What You Will Bring (Required) Bachelor’s degree required; preferably in social work, human services, or a related field. LSW or LISW in state of employment preferred. At least one (1) year of experience working with elders or adults with disabilities. Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required. Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting. Training, evaluating, and supervising caregivers. Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs. Documenting in an accurate and timely manner; maintain a clinical record for each morbidity patients. Monitoring the health status of all members to ensure that all needed care is properly delivered. Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc). Additional state-specific qualifications may be required.
