The Care Manager provides compassionate emotional support, guidance, advocacy, and care coordination services to children and their families coping with serious medical conditions, chronic illnesses, disabilities, or emotional and behavioral challenges. Working closely with children, parents, siblings, healthcare providers, and community organizations, the Care Manager helps families navigate complex medical, educational, social, and emotional needs. The Care Manager conducts comprehensive assessments, develops individualized care plans, connects families with appropriate resources and services, and monitors progress to ensure positive outcomes. This role serves as a trusted advocate and partner for families, helping them access support systems that enhance their well-being and quality of life. The Care Manager is responsible for ensuring all clients receive accurate, professional, and compassionate guidance, referrals, advocacy, and support services aimed at achieving the best possible outcomes for children and their families. Requirement: This is a remote position; however, candidates must reside in New York State or surrounding States, as the role requires monthly in-person visits with clients throughout New York. Applicants must be able to meet these travel requirements. Work Schedule & Caseload Expectations · This position is engaged as an independent consultant/contractor and is compensated based on meeting established billable service requirements and timely documentation standards. · Consultants are expected to maintain a minimum caseload of five (5) active client cases. · This role offers flexible scheduling, with services provided based on client and family needs. · Availability may include evenings and weekends to accommodate family schedules and program requirements. · Consultants must maintain availability during at least one weekday business day to support client communication, care coordination, meetings, and collaboration with healthcare and community partners. · Consultants are responsible for meeting established billable productivity expectations while ensuring high-quality service delivery and documentation compliance. · Position requires the ability to travel within range and conduct home and community visits as needed. Education A bachelor's degree with 2 years of relevant experience OR A master's degree with 1 year of relevant experience About CLCF In partnership with the Collaborative for Children & Families (CCF), Chai Lifeline Children’s Fund (CLCF) draws upon the extensive experience and expertise of its affiliated organization, Chai Lifeline—an international nonprofit founded in 1987—to provide comprehensive emotional, social, and practical support to children, families, and communities facing serious illness, trauma, and loss. CLCF helps families navigate the many challenges associated with medical crises through personalized care coordination and family-centered support. Our Care Managers are experienced in working with chronic health conditions and medically fragile children and adolescents. Through education, advocacy, resource navigation, and ongoing support, we help families achieve better health outcomes and greater stability. Key Responsibilities Assess clients' eligibility for services and determine the level of support required. Conduct home, hospital, school, and community visits as needed. Build supportive relationships with children and their family members, providing ongoing emotional support and guidance. Complete comprehensive assessments of clients' medical, emotional, social, educational, and practical needs. Develop, implement, and update individualized care plans based on family goals and identified needs. Connect families with appropriate healthcare providers, community programs, social services, educational resources, and financial assistance programs. Coordinate communication among children, parents, caregivers, healthcare professionals, educators, insurance providers, and other service partners. Collect and maintain client information and documentation accurately and in a timely manner. Maintain detailed case notes and records of client and family interactions. Monitor client progress and evaluate the effectiveness of care plans, making adjustments as needed. Research and identify local, regional, and national resources that address the unique needs of children and their families. Advocate for children and families by facilitating access to services, making calls, writing correspondence, attending meetings, and assisting with problem resolution. Prepare reports and maintain case files to ensure compliance with organizational policies, program requirements, and audit standards. Complete all required documentation in accordance with funding, program, and regulatory guidelines. Support families during challenging situations, helping them navigate healthcare systems and other service networks. Participate in team meetings and collaborate with internal and external stakeholders to promote comprehensive care. Assist with special projects and other duties as assigned. Skills, Knowledge and Expertise Outstanding verbal and written communication skills, with the ability to engage effectively with children, families, healthcare professionals, and community partners. Highly resourceful with the ability to research, develop, and leverage local, regional, and national resources to address the unique and evolving needs of children and their families. Strong interpersonal skills with a compassionate, family-centered approach. Excellent organizational and case management skills. Demonstrated ability to manage multiple priorities and meet deadlines in a fast-paced environment. Strong problem-solving and advocacy skills. Proficiency in Microsoft Office Suite, Google Workspace, and case management systems; ability to learn new software applications as needed. Experience handling confidential and sensitive information with professionalism and discretion. Ability to work independently and collaboratively as part of a multidisciplinary team. Knowledge of healthcare, social service, educational, and community resources systems for children preferred. Experience working with children, families, and/or individuals facing medical, developmental, behavioral, or emotional challenges preferred.
