The Care Manager Supervisor provides leadership, supervision, and support to Care Managers while maintaining a reduced caseload of children and families facing serious medical conditions, chronic illnesses, disabilities, trauma, or emotional and behavioral challenges. This role ensures the delivery of high-quality, compassionate, family-centered services through clinical oversight, case consultation, staff development, and direct service provision. The Care Manager Supervisor is responsible for overseeing assessments, care planning, resource coordination, advocacy efforts, documentation compliance, and program outcomes. Working collaboratively with families, healthcare providers, educators, community agencies, and internal team members, the Supervisor ensures that all clients receive comprehensive support and that Care Managers are equipped to effectively address complex family needs. Requirement: This is a remote position; however, candidates must reside in New York State or surrounding States, as the role requires monthly in-person visits with clients throughout New York. Applicants must be able to meet these travel requirements. Education Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Public Health or a related field required. Master's degree in a related field preferred. About CLCF In partnership with the Collaborative for Children & Families (CCF), Chai Lifeline Children’s Fund (CLCF) draws upon the extensive experience and expertise of its affiliated organization, Chai Lifeline—an international nonprofit founded in 1987—to provide comprehensive emotional, social, and practical support to children, families, and communities facing serious illness, trauma, and loss. CLCF helps families navigate the many challenges associated with medical crises through personalized care coordination and family-centered support. Our Care Managers are experienced in working with chronic health conditions and medically fragile children and adolescents. Through education, advocacy, resource navigation, and ongoing support, we help families achieve better health outcomes and greater stability. Key Responsibilities Staff Supervision and Program Leadership Provide direct supervision, coaching, and ongoing support to Care Managers and other direct service staff. Conduct regular case reviews and consultations to ensure quality service delivery and positive client outcomes. Review and monitor client documentation, assessments, care plans, and case notes for accuracy, completeness, and compliance. Assist staff in developing intervention strategies for complex cases and crisis situations. Manage intake and referral processes, assess client eligibility, and assign cases based on staff capacity and expertise. Identify emergency service needs and coordinate crisis response efforts. Monitor staff productivity, caseloads, and performance expectations. Support training, onboarding, professional development, and continuous quality improvement initiatives. Participate in strategic planning, program evaluation, and special projects as assigned. Direct Care Management Maintain a caseload of children and families requiring care coordination and support services. Conduct comprehensive assessments of medical, emotional, social, educational, and practical needs. Develop, implement, and update individualized care plans in partnership with families and service providers. Provide emotional support, guidance, and counseling to children and family members navigating illness and life challenges. Coordinate communication among families, healthcare providers, schools, insurance companies, and community partners. Connect families with healthcare services, educational resources, financial assistance programs, and community-based supports. Advocate for families through meetings, correspondence, service navigation, and problem resolution. Conduct home, hospital, school, and community visits as needed. Monitor client progress and adjust care plans to ensure services remain responsive to family needs. Maintain timely and accurate documentation, reports, and case records in accordance with organizational, funding, and regulatory requirements. Assist in ad hoc assignments Skills, Knowledge and Expertise Experience Minimum of 3-5 years of experience in care management, case management, social services, healthcare navigation, or a related field. Previous supervisory, team lead, or mentoring experience preferred. Experience working with children, families, and individuals facing medical, developmental, behavioral, or emotional challenges strongly preferred. Knowledge, Skills, and Abilities Strong leadership, coaching, and team development skills. Outstanding verbal and written communication abilities. Excellent organizational and time management skills with the ability to manage multiple priorities. Strong assessment, problem-solving, and critical-thinking skills. Demonstrated ability to provide effective advocacy and resource navigation. Ability to manage sensitive and confidential information with discretion. Proficiency in Microsoft Office Suite, Google Workspace, electronic records systems, and case management platforms. Ability to work independently while collaborating effectively within multidisciplinary teams. Knowledge of healthcare systems, social services, educational resources, and community-based support programs. Commitment to culturally responsive, trauma-informed, and family-centered care.
