Care Partner – Work from Home

Do meaningful work, build genuine client relationships and have a manager who genuinely has your back.

What if your next job gave you the flexibility to work from home, a manager who trusts you to get the job done, and a team that actually supports each other?

We’re looking for an experienced Care Partner to join our team in a work-from-home role, supporting older Australians to live independently and comfortably in their own homes.

And honestly? This is the kind of job where the manager makes all the difference.

A Manager Who Trusts You

Your manager is approachable, hands-on and genuinely supportive.

They have an open-door approach — even when that door is virtual — and want you to feel comfortable asking questions, raising concerns or bringing forward ideas.

They take the time to know their people: how they work, what motivates them, their strengths and where they might need some extra support.

You'll get guidance when you need it and autonomy when you've got it.

No micromanaging. No unnecessary checking in. No feeling like someone is constantly looking over your shoulder.

You'll be trusted to manage your clients, your workload and your day — while knowing your manager is there when you need them.

And when the team needs extra support, they're not afraid to roll up their sleeves and get involved.

Work from home — and still feel part of a team

Working from home doesn't mean working alone.

You'll have the flexibility and convenience of working from your own home while being part of a friendly, collaborative team that genuinely helps each other out.

We're looking for someone who enjoys the independence of working remotely but also knows how to stay connected, communicate and take ownership.

If you're someone who can organise your own day, manage competing priorities and get things done without needing to be chased — you'll love this environment.

Clients first. Results matter too.

At the heart of this role are your clients.

You'll build genuine relationships, understand what matters to each person and coordinate the services they need to live well at home.

You'll also be responsible for bringing new clients on board — with a target of around two new clients per week — completing the associated paperwork and maintaining regular contact with every client through monthly monitoring calls.

Because we believe great client outcomes and commercial sustainability go hand in hand.

We're passionate about providing exceptional care, but we're also a business. Meeting our KPIs means we can continue growing, supporting our clients and creating sustainable jobs for our people.

So if you're client-focused and commercially aware, you'll fit right in.

What You'll Be Doing

You'll be responsible for:

  • Building strong, genuine relationships with clients and their families.
  • Understanding client needs, goals and preferences.
  • Developing and coordinating personalised care plans.
  • Coordinating services and providers to make sure clients receive the right support.
  • Advocating for clients and making sure their voice is heard.
  • Managing Home Care Package budgets responsibly.
  • Bringing new clients into the service and completing the required onboarding and documentation.
  • Completing monthly monitoring calls to check that clients are happy and their services continue to meet their needs.
  • Identifying changes in client needs and coordinating appropriate responses.
  • Keeping on top of your documentation, administration and KPIs.
  • Working collaboratively with your manager and wider team.

The role is about care coordination, relationships and client outcomes — not providing hands-on nursing care.

Who are we looking for?

You Could Be a

Support Worker ready to take the next step.

Care Coordinator or Case Manager wanting a role with more autonomy.

Aged Care professional who understands what great support looks like.

Case Manager from another provider who knows how to manage clients, budgets and services.

Nurse looking to move into care coordination — provided you're looking for a role that isn't a clinical nursing position.

A Certificate IV or higher in aged/community care, care coordination or equivalent experience is required, along with experience in case management, wellness/reablement or aged care service delivery.

You'll be a great fit if...

  • You genuinely care about people and the outcomes they achieve.
  • You're organised and don't need someone chasing you.
  • You take ownership of your work.
  • You're comfortable working to KPIs and achieving targets.
  • You're commercially aware and understand why sustainable growth matters.
  • You're confident working independently from home.
  • You're friendly, collaborative and happy to help your team.
  • You communicate openly and ask for help when you need it.
  • You enjoy building relationships rather than simply processing paperwork.

Why this could be your next favourite job

  • Work from home
  • A manager who trusts and supports you
  • Meaningful work with real impact Clear expectations and achievable KPIs
  • A team that genuinely helps each other
  • Autonomy to manage your day
  • The opportunity to build lasting client relationships

If you want flexibility without losing connection, autonomy without being left on your own, and a manager who genuinely has your back — this could be the role you've been looking for.

Come join a team where you're trusted to do your job, supported to succeed and able to make a real difference every day.
