Salary Range: $26.00 To $27.30 Hourly POSITION PROFILE: Do you love working with fathers and guiding them to be the best parents they can be? If so, the Case Manager I position may be the next position for you! Under the Supervision of SAY San Diego’s Dads Corps Program Director and Program Manager, the Case Manager is responsible for the support and coordination of clients in the federally funded fatherhood program, SAY San Diego’s Dads Corps. The Case Manager will be experienced in direct client services, family support services, multi-disciplinary case management, community development, and supervision. This position requires independent judgment, creativity, and adaptability. This position is eligible for a 60/40 hybrid work schedule and is Monday through Friday with the flexibility to work after hours or weekends when applicable. ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES: Provide case management services through client-centered approach with the On My Shoulders Curriculum. Manage case load of approximately 10 fathers per cohort to include intake , client needs assessment, goal setting, develop individual service plans, advocacy for client development, and follow-up services. Maintain accurate client files, document case notes in timely manner, and complete funder reports as requested Schedule and conduct home or in-person visits with clients for intake and or client servicing appointments Coordinate and conduct group facilitation may include trauma-informed services, leadership development, socio-emotional curriculum, or other topics of interest. Develop and maintain positive relationships with clients and other connected program partners. Prepare and facilitate Dads Corps class sessions on parenting, healthy relationships, and/or economic stability. Maintain regular communication with clients through various mediums (in-person, over the phone, and electronic communication) from participant enrollment to program exit activities. Prepare and facilitate evidence-based, interactive, and engaging workshops to deliver curricula with fidelity to build alumni base. Develop a network of key organizational partners to support clients when faced with a crisis and/or support necessary to reach personalized goals. Provide on-going support for clients requiring additional support including life skills training, family building, legal resources, job readiness, etc. Support the outreach, recruitment, and retention of program participants. Attend and participate in partner and funder-related meetings, as required. QUALIFICATIONS (Education, Experience, and Certifications): Associate’s degree in social work or related social science/field preferred, or 2 years equivalent case management work experience. Experience in case management, group facilitation, direct client service, and delivering curriculum-based interventions. Demonstrated experience working with diverse populations and cultures and socio-economic families Bi-lingual English/Spanish preferred KNOWLEDGE, SKILLS, AND ABILITIES: Ability to demonstrate knowledge of trauma-informed care and practices. Solid proficiency of Microsoft Office to include Word, Excel and Outlook. Accurate data entry of time sensitive information in fast paced environment. Strong communication skills - ability to document client services in clear and concise manner. Ability to effectively manage multiple demands and time-sensitive priorities. Works well with a broad range of diverse, cultural and socio-economic populations and backgrounds. Possess an awareness of multi-cultural critical issues and community conditions facing children, youth, and families throughout San Diego Access to reliable transportation and willing to use it for work-related purposes.
