Join a compassionate and collaborative team delivering person-centred support to individuals with alcohol and other drug (AOD) use and complex needs across the Central Coast. As a CCCP Care Coordinator , you will provide intensive case management and specialist support to people experiencing alcohol and other drug (AOD) use alongside other complex needs. You'll build trusted relationships with clients, complete comprehensive assessments and develop personalised care plans that empower individuals to achieve their goals. Based in Gosford, you'll deliver support through outreach, office-based appointments and virtual services while working closely with internal teams and community partners to achieve lasting outcomes. Your Responsibility Deliver person-centred assessments and tailored care plans Provide intensive case management and specialist AOD support Collaborate with internal teams, referrers and external service providers Build and maintain strong referral pathways across health and community services Deliver culturally safe and appropriate support for Aboriginal and Torres Strait Islander clients Support clients across housing, employment, education, daily living, social connection and family reunification Maintain accurate case notes, records and reporting requirements You Will Need ✔ A relevant tertiary qualification or VET qualification with substantial industry experience ✔ Experience supporting people experiencing AOD use, mental illness, trauma, homelessness or justice system involvement ✔ Skills in completing assessments and delivering therapeutic interventions ✔ A professional, ethical and non-judgemental approach ✔ The ability to work both independently and collaboratively ✔ Knowledge of local health and community support services ✔ Confidence using Microsoft Office and client management systems ✔ A current NSW Driver Licence Apply Now If this sounds like you , don’t wait - apply now , we are interviewing applications as we receive them.
