Child Development Care Coordinator Hot Job Fully Remote • Headquarters - Alameda, CA 94501 Apply Overview Salary Range $77,000.00 - $112,000.00 Salary Category Health Care Apply Description Provides direct support to Alameda County families by connecting them with services and community resources that promote their children’s development. Assesses family needs and eligibility, responds to referrals and inquiries, and manages assigned cases. Respond to a high volume of calls and referrals from the Help Me Grow (HMG) Phone Line, supporting children’s development and family well-being. Talk with families about developmental, behavioral, and other family needs; create child-centered care plans and help families understand and support their child’s development. Connect families with appropriate community resources, services, benefits, and other supports, including HMG Family Navigation and partner agencies. Assess family needs and eligibility for services and coordinate referrals to resources that address health, developmental, and social needs. Work with service providers to discuss developmental concerns, referral options, and available community resources. Follow up with families and service providers to monitor progress, confirm connections to services, and provide additional support as needed. Maintain accurate program data, identify barriers to accessing services, and work with HMG staff and community partners to improve service delivery. Build and maintain strong relationships with community agencies to ensure families receive coordinated and timely support. Qualifications Minimum Qualifications Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. Additional experience and/or education can be substituted to meet typical qualifications. A typical way to obtain the required qualifications would be: 3 years increasingly responsible experience in child development, social service, or psychology supplemented by 2 years college-level coursework or training in child development, social work, psychology, or a closely related field. *A college degree is not required. Additional relevant work experience may be substituted to meet the college coursework standard. Bilingual proficiency is strongly preferred. Knowledge of: General knowledge of child development (typical and atypical), developmental disabilities, infant and early childhood mental health, family support, and the social determinants of health. Principles and practices of assessment and early intervention of children birth to age five. Social service, health, education and developmental agencies and resources in Alameda County. Entitlement systems for children birth to age five and their families, including eligibility requirements. Outreach, interviewing, and care coordination/navigation skills and techniques. Strategies for effective problem-solving and identification of systems issues. Applicable federal, state, and local laws, codes, and regulations as well as industry standards and best practices pertinent to the assigned area of responsibility. Agency and mandated safety rules, regulations, and protocols Techniques for providing a high level of customer service, by effectively dealing with the public, vendors, contractors, and Agency staff. The structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammar. Modern equipment and communication tools used for business functions and program, project, and task coordination, including computers and software programs relevant to work performed. Ability to: Use the telephone as the primary mode of communication, work effectively with and outreach to ethnically and culturally diverse families. Promote basic principles of health promotion and preventative health care with an understanding and acceptance of differences in attitudes toward health care and services. Gain confidence of and obtain personal and confidential data from clients, work with and safeguard confidential information. Effectively represent the department and the Agency in meetings with governmental agencies; community groups; various business, and professional organizations; and in meetings with individuals. Understand, interpret, and apply all pertinent laws, codes, regulations, policies and procedures, and standards relevant to work performed. Prepare clear and concise correspondence, documentation, and other written materials. Use tact, initiative, prudence, and judgment within general policy and procedural guidelines . Organize work, set priorities, meet critical deadlines, and follow-up on assignments. Communicate clearly and concisely, both orally and in writing, using appropriate English grammar and syntax. Establish, maintain, and foster positive and effective working relationships with those contacted in the course of work. Effectively use computer systems, software applications relevant to work performed, and modern business equipment to perform a variety of work tasks. Observes and complies with all Agency and mandated safety rules, regulations, and protocols. Please note: This position is designated as full remote, but employees come to the office to attend meetings and as requested. All First 5 employees must live and work in California. The salary range for this position is $77,500 to $112,000 and as an independent government agency, the posted salary is the set salary range for this position. We cannot offer salaries above this posted range. We conduct reference and background checks for all potential employees.
