The Clinical Case Manager – Adoption Support and Preservation (ASAP) program provides healing-centered, family systems-based therapeutic services to adopted children and their families, with the goal of maintaining placement and improving the overall well-being and attachment of the child within the home environment. As a Clinical Case Manager, you will assess the behavioral health needs of participants in community-based and/or grant-funded programs, provide clinical assessments as required by program scope, and provide or recommend appropriate therapeutic interventions. This position will carry a partial caseload and provide ongoing resources, support, and linkages to services for families participating in the program. The Clinical Case Manager will also actively participate in community cohorts and build relationships within the community to increase awareness of ASAP services, strengthen referral partnerships, and generate referrals to the program. Core Responsibilities and Essential Job Functions: Maintains a caseload or provides services in collaboration with another assigned provider to participants whose needs range from routine to complex and provides a range of therapeutic services, utilizing social work principles and theory in treatment of participants. Assesses participant(s)’ situation, strengths, behavioral health, and functioning using a mental health assessment. Formulates treatment plans and goals with assigned participants(s), obtains supervisory approval of the plan, and is responsible for the implementation of the plan. Engages program participants in therapeutic interventions which may include individual, family, and/or group therapy; psychoeducation; and/or other supplemental interventions, in accordance with program model. Refers program participants for additional recommended services if unable to provide within the program scope. Analyzes and evaluates progress in the treatment plan and modifies when necessary to meet the needs and goals of the participant(s). Assumes responsibility for comprehensive case management which includes needs assessment, service planning, case coordination, service brokerage, linkages with community resources, monitoring progress toward participant(s)’ goals and objectives, and makes recommendations for on-going services when necessary. Coordinates services and attends meetings with other agencies, systems, and community providers to meet the needs of the participant(s). Schedules appointments to the participant(s)’ home or a setting most agreeable to the participant(s) at a time that is convenient for involved parties. Provides and/or arranges transportation for participants. Responds to crisis calls and provides clinical management of the crises. May participate in an after-hours on-call rotation. Develops aftercare treatment plans with the participant(s) and ensures that any recommended services or providers are in place prior to participant(s) being discharged. Reports any suspected abuse or neglect to the Department of Children and Family Services per the Abused and Neglected Child Reporting Act. Attends regular supervision staffing's with designated program supervisor and attends clinical supervision with clinical team when deemed necessary. Attends community events which may occur outside of regular business hours. Writes and maintains complete documentation of services for each case in paper and digital form and meets program record-keeping requirements. Candidate qualifications : Master’s Degree in Social Work or a related field preferred or Bachelor's Degree in Social Work or a related field with at least two years of relevant experience required. Valid driver's license, insurance, and a reliable vehicle required. The ability to communicate effectively in both English and Spanish strongly preferred. Knowledge of resources in the Cook County area strongly preferred. Job details: Compensation: Salary Range $ 54,361-$56,000 ; offers are commensurate with experience and bilingual candidates may receive additional financial compensation. The salary range provided represents our current estimate for this role at the time of posting and is subject to change. The final salary will be determined based on a range of factors, including but not limited to, a candidate's relevant experience, education, qualifications, skills, certifications, and alignment with organizational needs.
