Description Position: Pharmacist  The Pharmacist supports Monogram’s medication optimization program by conducting comprehensive medication reviews, identifying drug therapy concerns, and communicating clinical recommendations to treating providers. The pharmacist conducts telephonic consultations with medical providers when medication changes appear necessary and speaks with patients about medication use concerns. The pharmacist collaborates with interdisciplinary clinical staff to integrate medication optimization into Monogram's care model and ensures that field clinicians are executing goal-directed medical therapy (GDMT) policies and procedures. This role involves conducting telephonic consultations with medical providers when medication changes appear necessary and engaging directly with patients on medication use and adherence. The Pharmacist serves as the primary pharmacy resource for their Monogram clinical team. Roles and Responsibilities  Perform and write concise patient-specific comprehensive medication reviews (CMRs); productivity and quality standards apply. Drive meaningful medication interventions that support medical providers and patients in adhering to evidence-based medication guidelines. Review patient treatment plans, clinical visit documentation and other medical records as needed to develop medication action plans, support targeted clinical outreach programs, and assist with quality assurance. Conduct telephonic consultations with treating providers when medication changes are clinically indicated. Engage patients directly to improve medication use, adherence, and understanding of their drug therapy. Serve as a pharmacist resource for Monogram field clinicians on medication use and optimization across complex polychronic conditions. Partner with Monogram clinicians to deliver medication optimization training that builds a standardized, effective GDMT program for clinical staff. Monitor important medication availability and use trends for populations under our Monogram provider's care. Maintain current knowledge of evidence-based prescribing for individuals with polychronic medical conditions with specific focus on advance cardiovascular, endocrine, and kidney diseases. Participate in internal interdisciplinary team case rounds or meetings and produce medication-related documentation to be shared with treating providers. Implement medication optimization processes that meet the needs of Monogram partners and the patients they serve. Support Monogram as necessary with any tasks required to deliver excellent personalized medical (physical and mental health) care and perform all other duties as assigned Uphold the mission and values of Monogram Health in all interactions with patients, providers, and colleagues. Position Requirements   Remote positions located within communities that Monogram Health serves with some domestic travel Bachelor degree in Pharmacy or Doctor of Pharmacy (PharmD) Current, unrestricted pharmacist license in state of service Minimum Three years of active pharmacist practice experience, preferably in a managed care, ambulatory care, or clinical patient care environment Desired qualification include; PGY1 residency in clinical fields Board certification in Medication Management or a related clinical field MTM certificate training from APhA, PowerPak, or similar program Ability to work from a professional, remote location in area of service or assignment Monday through Friday, 8:00 AM to 5:00 PM local time, and as business needs require Benefits  Opportunity to work in a dynamic, fast-paced and innovative value-based provider organization that is transforming the delivery of kidney care  Comprehensive medical, dental, vision and life insurance  Flexible paid leave and vacation policy  401(k) plan with matching contributions   About Monogram Health  Monogram Health is a next-generation, value-based chronic condition risk provider serving patients living with chronic kidney and end-stage renal disease and their related metabolic disorders.
