Clinical Pharmacist - Home Infusion

As a frontline clinical pharmacist in a rapidly scaling specialty program , you will manage complex patients, guide therapy outcomes, and help shape the future of home-based infusion care. The Role In this role, you will serve as a clinical leader within a high-growth specialty home infusion program , managing a dedicated panel of complex patients while collaborating across a multidisciplinary care team. This is a non-dispensing, clinically focused position that emphasizes patient outcomes, coordination, and program development. On a typical day, you will: Manage a panel of approximately 50 to 60 specialty infusion patients , supporting them from therapy initiation through stabilization Provide ongoing clinical monitoring , evaluating therapy effectiveness, side effects, and adherence for high-cost, high-complexity medications Conduct proactive patient outreach and education through a remote, telephonic care model, building trusted longitudinal relationships Serve as a central liaison between patients, providers, and pharmacy teams , ensuring seamless coordination of care Collaborate closely with physicians, clinic pharmacists, and internal teams to adjust therapies and resolve complex clinical scenarios Oversee refill coordination and therapy continuity , ensuring safe and timely medication access Participate in weekly process improvement and program development initiatives , contributing to workflow design and policy creation Support operational planning such as high-value medication ordering and forecasting , ensuring alignment with patient needs This role offers a high-touch patient experience without bedside responsibilities , allowing you to deeply engage in clinical decision-making while maintaining a structured and sustainable work environment. MINIMUM REQUIREMENTS: Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required . Experience: No experience required . PGY-1 residency or equivalent clinical experience preferred. Licensure: Virginia State Board of Pharmacy License or within 60 days of hire. American Heart Association (AHA) Health Care Provider BLS certification required . PHYSICAL DEMANDS: Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease. The pay range for this role is $64.37 - $87.33 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
