Overview Clinical Pharmacist - VytlOne Specialty-Remote VytlOne is currently seeking an experienced Clinical Pharmacist to join our growing Specialty Pharmacy in Lubbock, TX. This is an excellent opportunity for a Pharmacist to make a positive impact in the lives of the patients we care for, with a devoted pharmacy operations team. Job Summary The Clinical Pharmacist provides clinical pharmacy services for specialty pharmacy patients and internal teams in accordance with applicable laws, regulations, accreditation standards, policies, and procedures. This role reviews medication therapy for safety and appropriateness, supports prior authorization and utilization review activities, provides patient counseling and drug information, documents assessments and care plans, collaborates with providers and internal teams, and supports quality improvement, compliance, and operational initiatives. The Clinical Pharmacist may also dispense prescribed medications and oversee pharmacy technician activities as permitted by law and assigned by leadership. Salary is $150K-$155K DOE and specialty certification Responsibilities ESSENTIAL FUNCTIONS (These duties must be performed with or without reasonable accommodation. This document in no way states or implies that these are the only duties to be performed by the employee occupying this position): Interpret and fill drug orders accurately and timely Ensures the appropriateness of all medication orders, new prescriptions or refills, in reference to the client’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of the active medication profile, provider notes, and available pertinent laboratory information. Conduct/Review Drug Utilization Reviews to check for drug duplication, interactions or allergies. Review non-formulary drug requests using predetermined criteria. Review prior authorization requests using predetermined criteria. Maintain current knowledge of drug therapies and drug reactions and interactions by reading professional literature, attending seminars and through professional affiliations. Contact and confer with physicians regarding questions or irregularities with prescription practice whenever safety or established alternatives have not been utilized. Monitor drug therapy for contraindications, interactions, allergies and appropriateness. Documents / maintains initial patient assessments, reassessments and care management plans in the electronic patient management program, e.g., Asembia, CPR Plus. Provide drug information to meet the needs of medical and nursing staff members through direct contact or in-service programs. Provide patient information and counseling. Participate as an active member and facilitator in process improvement teams. Prepare provider newsletters or articles to keep providers updated. Provide individual and group training, in-services and other consultations to unit staff and providers to optimize therapy. Work with medical directors and staff in developing and carrying out various clinical initiatives. Generate and review standard monthly and quarterly reports, as required. Attend and participate in other programs, committees, meetings, and functions as required. Assist in the development of clinical, utilization management, ad hoc and production reports and programs. Assist in formulary, rebate analyses and pharmacoeconomic studies and initiatives. Perform retrospective drug utilization review: identify drug, physician, and patient utilization trends; make interventions as appropriate; monitor and document outcomes. Assist in the generation, maintenance, and distribution of formulary alternative lists, maintenance drug lists, and new drug to market lists. Work cooperatively with other staff members to enhance the team concept. Supervise pharmacy technicians in the routine performance of their duties. Participate in the development and implementation of the Performance Improvement Program. Assist in overseeing clinical procedures. Complete facility audits as assigned. Maintain regular attendance in accordance with established policies and procedures. Perform other job-related duties as assigned. Promptly report any allegations of impropriety to the Compliance Department. Comply with VytlOne’s Ethical Business Conduct policy and VytlOne’s Compliance Program. Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists. Complete required training, as assigned, within the established timeframes. Qualifications Experience : Residency trainimg or 2 years post-graduate pharmacist experience in specialty pharmacy, ambulatory care, managed care, infusion, retail, hospital, or another clinically relevant pharmacy setting preferred. Experience conducting clinical review, patient counseling, medication therapy assessment, prior authorization support, care plan documentation, or interdisciplinary collaboration strongly preferred. Education : BS or Pharm. D. from an accredited school of pharmacy required. Current, unrestricted pharmacist license in the employee’s primary state of practice required, with ability to obtain and maintain additional state pharmacist licenses as business needs require. Licenses: May be required to obtain and maintain additional state pharmacist licenses, including non-resident licenses, based on patient location, pharmacy operations, payer requirements, or business needs. Certifications: Specialty pharmacy, immunization, medication therapy management, sterile compounding, or other role-related certifications may be preferred or required based on assigned duties, pharmacy services, and applicable regulatory requirements. Knowledge, Skills and Abilities: Knowledge of federal and state pharmacy laws, clinical pharmacy practice standards, specialty pharmacy workflows, medication safety principles, drug utilization review, prior authorization processes, patient counseling requirements, clinical documentation standards, and applicable accreditation, regulatory, and payer requirements. Ability to evaluate medication therapy for appropriateness, safety, contraindications, interactions, duplications, adherence concerns, and other patient safety risks, and to communicate evidence-based recommendations clearly to providers, patients, caregivers, and internal teams. Proficiency using pharmacy management systems, electronic patient management platforms, Microsoft Office applications, secure communication tools, and other technology required to support remote specialty pharmacy operations. Ability to organize, prioritize, and manage clinical, operational, and documentation tasks to meet patient care, provider, payer, compliance, and business timelines. Ability to use available clinical resources, policies, procedures, systems, and team support effectively to resolve patient, provider, payer, and operational needs. Ability to communicate clearly, professionally, and compassionately with patients, caregivers, providers, pharmacy team members, payers, and other internal and external stakeholders, including when explaining clinical recommendations, documentation needs, coverage requirements, or sensitive patient information. Ability to learn and use job-related software, pharmacy systems, electronic patient management platforms, secure communication tools, and reporting applications accurately and efficiently. Ability to maintain accurate, timely, and complete clinical documentation, patient records, intervention notes, care plans, and required pharmacy records in accordance with company policy, documentation standards, and applicable regulatory, accreditation, and payer requirements. Ability to evaluate clinical and operational workflows, identify opportunities for improvement, support policy and procedure implementation, and contribute to strategies that improve quality, compliance, patient care, and operational effectiveness. Knowledge of quality improvement principles, performance improvement activities, audit readiness, outcome monitoring, and procedures used to support safe, compliant, and effective specialty pharmacy services.
