ROLE OVERVIEW and PURPOSE: The Clinical Pharmacy Services Nurse (RN) works to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic refill team environment. This role will perform nursing-related assessments to provide appropriate pharmacy care to patients. *Please note, this opportunity is hybrid and consists of 8 hours on-site and 32 hours remote per week. The regular schedule is Monday, Tuesday, Thursday, and Friday from 6:30 AM to 3:00 PM remotely, and Wednesday from 7:30 AM to 4:00 PM on-site.* RESPONSIBILITIES: Learns and navigates Neighborhoods electronic medical record (EMR) to investigate refill requests initiated by patients and pharmacies Communicates via the EMR with other Neighborhood staff related to patient care in accordance with the Neighborhood Pharmacist-Nurse Refill Protocol Refills medications on behalf of a provider according to Neighborhood’s Pharmacist-Nurse Refill Protocol Orders labs needed to monitor medications on the Neighborhood Pharmacist-Nurse Refill Protocol Orders outstanding routine labs required for health maintenance Reviews current medication lists to identify duplicate or missing medications on patient medication lists Contacts patients and pharmacies to clarify medication discrepancies prior to refilling a medication Maintains accurate and complete records in the electronic health records of all patient encounters Attends and participates in required meetings and trainings Performs all job functions with the neighborhood’s mission, vision, values, and strategic plan in mind Duties Specific to Registered Nurse Modifies the directions of a current medication when directed by a Clinical Pharmacist or provider Modifies the strength and formulation of a current medication if the total daily dose remains the same Substitutes therapeutically equivalent current medications when directed by a Clinical Pharmacist or provider Send new prescriptions when the medical record is clear that the provider documented the name of the medication, strength, and directions for use Reviews telephone encounters for abnormal electrolyte values to determine if medications are safe to refill or require provider review Clarifies medication discrepancies for refill center staff when there are duplicate or missing medications on the patient’s medication list Quality Management Contributes to the success of the organization by participating in quality improvement activities Cooperate in accomplishing department goals and objectives Customer Relations Responds promptly and with caring actions to patients and employees Maintains professional working relationship with all levels of staff, patients, and the public EDUCATION/EXPERIENCE: Associate's degree in nursing required Bachelor's degree in nursing preferred Active CA RN license required One year hospital, clinic, outpatient or general medicine experience required Current Basic Life Support (BLS) certification though an American Heart Association (AHA)- Approved source is required upon hire and must be maintained as a condition of employment. AHA- approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques. Online-only BLS courses without a live skills check do not meet this requirement. ADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities): Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills Ability to interpret a variety of instructions in written, oral, diagram, or schedule form Knowledgeable about and experience with pharmacy practices and protocols Ability to successfully manage multiple tasks simultaneously Excellent planning and organizational ability Ability to work as part of a team as well as independently Ability to work with highly confidential information in a professional and ethical manner Ability to follow the NHC Pharmacist-Nurse Refill Protocol for refill authorizations Physical Requirements Ability to lift/carry 20lbs/weight Ability to stand for long periods of time Pay Range : $45.91 - $67.93 per hour, depending on experience Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
