Job Summary The Community Health Team Supervisor leads and supports a team dedicated to improving patient outcomes through culturally responsive outreach, preventive care, and chronic disease management. This role supervises community health staff, partners with community organizations and health centers, and helps patients access screenings, preventive services, and community resources. The Supervisor also leads workforce development and implementation initiatives across the network, providing training, workflow design, data analysis, and strategic guidance to community health partners. By strengthening care coordination, population health efforts, and value-based care initiatives, this role helps improve quality measures, including cancer screening rates, blood pressure control, and diabetes management, while enhancing patient experience and reducing barriers to care. What You’ll Do Provide training, coaching, and technical assistance to community health centers (CHCs) to support outreach programs, quality improvement initiatives, and workforce development efforts. Analyze outreach and engagement data, monitor program performance, and support grant compliance and reporting requirements. Supervise, mentor, and develop Community Outreach Specialists and related staff, including recruitment, onboarding, performance management, and professional development. Collaborate with internal and external partners to develop workflows, policies, and best practices that improve outreach operations, care coordination, and service delivery. Support patients in accessing preventive care and managing chronic conditions through education, care navigation, appointment coordination, and barrier resolution. Identify and address social and logistical barriers to care, connect patients with community resources, and support referral follow-up and documentation. Participate in interdisciplinary care teams, quality improvement activities, and program planning to enhance population health outcomes and patient experience. Ensure compliance with organizational policies, HIPAA requirements, patient safety standards, and grant-funded program expectations. What We’re Looking For Required Qualifications: High school diploma or GED required; Associate degree or relevant certification preferred. Three to five years of experience in community health, care coordination, outreach, population health, or public health programs, including experience leading community health worker activities. Previous supervisory or team leadership experience. Experience working with diverse, underserved, and community-based populations. Basic understanding of medical terminology and healthcare systems. Proficiency with electronic health records, care management platforms, and mobile documentation tools. Strong organizational, communication, and problem-solving skills. Experience with writing, organizing, reporting, and presenting data from operations and/or research initiatives Preferred Qualifications: Colorado Department of Public Health & Environment Community Health Worker (CHW) Registry credential or ability to obtain the credential within a designated timeframe. Experience performing or supporting basic clinical screening activities, including blood pressure measurement or point-of-care testing. Experience with community organizing, community engagement, or community-based participatory research (CBPR). Knowledge of existing community resource networks. Bilingual or multilingual proficiency reflective of the communities served.
