Compensation: $50,000–$55,000/year Entry-level candidates welcome Annual salary increases Growth and advancement opportunities What You'll Do: Conduct community outreach and health screenings Support Medicaid members and individuals with asthma or chronic health conditions Complete health assessments and questionnaires Conduct home visits (approximately 8 per month; some virtual) Connect community members with resources and health-related services Assist with environmental health initiatives and supply coordination Present educational workshops and community programs Maintain accurate records across multiple systems Requirements: Fluent in Spanish and English (Required) Must live in Staten Island, NY or within a few mile radius.
