Community Health Worker (Bilingual Spanish) 📍 Staten Island, NY | 🕒 Monday–Friday, 8:00 AM–5:00 PM | ⭐ Direct Hire Make a Difference in Your Community! This is an excellent opportunity for someone passionate about community outreach, public health, and helping others. Entry level candidates are encouraged to apply! Compensation: $50,000–$55,000/year Entry-level candidates welcome Annual salary increases Growth and advancement opportunities What You'll Do: Conduct community outreach and health screenings Support Medicaid members and individuals with asthma or chronic health conditions Complete health assessments and questionnaires Conduct home visits (approximately 8 per month; some virtual) Connect community members with resources and health-related services Assist with environmental health initiatives and supply coordination Present educational workshops and community programs Maintain accurate records across multiple systems Requirements: Fluent in Spanish and English (Required) Must live in Staten Island, NY or within a few mile radius.
