The Specialty Care Patient Solutions team operates a patient advocacy support program spanning two critical therapeutic areas – Transthyretin Amyloid Cardiomyopathy (ATTR-CM) and Sickle Cell Disease (SCD) – designed to provide comprehensive care and innovative solutions that support patient journeys in both communities. Position Summary The Community Navigator (CN) is a remote, full-time role within the U.S. Patient Engagement Team, reporting to the Patient Engagement Director – ATTR & SCD as part of a national team of eight CNs (seven full-time territory-based CNs and one floater CN deployed across territories based on business need). The CN serves as a guide for individuals living with ATTR-CM – representing the majority (approximately 85%) of the role’s focus – and for individuals living with Sickle Cell Disease (approximately 15%), along with their caregivers, helping them understand the treatment landscape and access available resources. This dual-therapeutic-area remit requires the CN to build and maintain disease-state fluency, community relationships, and resource knowledge across two distinct patient populations with different patient journeys, advocacy ecosystems, and community needs. This field-based, community-facing position emphasizes patient education, support, and connection to local, regional, and national community partners in both the ATTR-CM and SCD communities. The CN is responsible for overseeing major programs, driving strategic initiatives, and working with cross-functional teams across both therapeutic areas. The CN has advanced expertise in the patient journey, which may inform organizational direction through key insights that contribute to long-term business objectives. The CN adheres to compliance & regulatory standards and fosters innovation to advance patient engagement. The CN is also accountable for measurable outcomes against defined KPIs spanning both therapeutic areas and plays a key role in shaping patient engagement strategies and solutions within the Specialty Care organization. The CN does not assist with reimbursement support for Pfizer ATTR-CM or SCD treatments but appropriately triages patients to reimbursement resources as needed. Key Responsibilities Patient Education and Support: Educate patients and caregivers about ATTR-CM and SCD, treatment options, local and regional support offerings, and health and wellness resources. Provide ongoing education and disease state awareness throughout the patient journey, tailored to the distinct needs of each community. Dual Therapeutic Area Coverage: Allocate time and effort across both therapeutic areas in line with team priorities (approximately 85% ATTR-CM / 15% SCD), maintaining current disease-state knowledge, approved resources, and community relationships in each; flex allocation as directed to meet evolving business and community needs. Patient Navigation: Assist patients in navigating care by connecting them to relevant resources within their communities and at Pfizer, recognizing the distinct care pathways, sites of care, and support networks for ATTR-CM and SCD patients. Health Equity: Engage thoughtfully with underserved communities, with particular attention to populations disproportionately affected by SCD, ensuring equitable access to education, resources, and support. Collaboration: Coordinate with Key Account Directors, management, and cross-functional partners across two brand teams to ensure patient goals and support objectives align with local market dynamics and patient journeys in each therapeutic area. Project Management: Set objectives, manage complex projects, and oversee ongoing activities within the Business Unit. Projects may include collaborations and partnerships with Healthcare Systems, HCPs/Educators, and Advocacy partners across both the ATTR-CM and SCD communities. Expertise and Knowledge Sharing: Serve as a subject matter expert in both therapeutic areas, adapting and implementing strategic actions. Share knowledge across the organization and contribute to knowledge management initiatives. Innovation and Solutions: Develop solutions to complex problems, champion innovation, and implement new ideas, processes, or products. Make decisions that impact current and future Business Unit objectives. Why Patients Need You Individuals with ATTR-CM and individuals with SCD – along with their caregivers – often face challenges in diagnosis, understanding treatment options, and maintaining therapies. Patients in the SCD community may face additional barriers related to access and long-standing health disparities. Proper education and awareness empower patients to engage in informed discussions with healthcare providers, especially as new therapies emerge. The CN is the central point of contact in their geography for both communities, focused on disease awareness, education, and connecting patients to resources both within Pfizer and in their local communities. What You Will Achieve Provide guidance and educational resources to address patient questions and information needs in both the ATTR-CM and SCD communities. Support patients in navigating their care by connecting them to appropriate resources. Share information on emerging treatments and the treatment journey to facilitate shared decision-making. Coordinate with advocacy and support teams in both therapeutic areas, attending events to present approved educational content. Use motivational interviewing to identify patient needs and tailor engagement to each community. Offer both in-person and virtual engagement for opted-in patients, utilizing print and digital resources. Build trust and provide ongoing support for patients to take informed next steps. Refer patients and caregivers to local health and wellness organizations. Develop solutions to complex problems, influencing long-term goals and outcomes. How You Will Have Impact Operate within a geographic region to educate and connect ATTR-CM and SCD patients to healthcare resources, balancing effort across both communities in line with team priorities. Strictly adhere to all regulations and policies, collaborating with legal and compliance teams and recognizing the distinct compliance considerations of each therapeutic area. Present the Community Navigator role and Pfizer support resources at local, regional, and national events across both the ATTR-CM and SCD communities. Partner with internal teams to meet patient needs and provide educational content to support groups in each therapeutic area. Engage with patients and caregivers through various communication channels. Provide local insights from both communities to inform the development of patient engagement materials and support resources. Organize travel and materials for events in a timely manner, ensuring thorough preparation. Minimum Qualifications B.S. or B.A. degree required. Minimum 7–10 years of pharmaceutical or related industry experience. ATTR-CM and/or Sickle Cell Disease experience Ability to build and maintain disease-state and community knowledge across more than one therapeutic area, and to prioritize effectively across concurrent responsibilities. Demonstrated exceptional communication, listening, and presentation skills. Proven ability to work in regulated environments and adhere to compliance guidelines. Track record of broad collaboration with diverse clients and stakeholders. Experience in responsible business practices and compassionate communication. Experience engaging diverse and underserved patient communities with cultural humility. Familiarity with Pfizer and industry compliance guidelines for working with Medical and Access & Reimbursement colleagues. Ability to deliver concise, meaningful presentations with integrity and empathy. Strong attention to detail, organizational, and business skills. Willingness to travel up to 50%, including weekends and evenings. Preferred Knowledge and Skills Clinical background preferred (RN, APRN, NP, ARNP, ACSW, LCSW, MPH). Knowledge of ATTR-CM and/or Sickle Cell Disease and patient/public advocacy. Experience with hematology, rare disease, or cardiology patient communities. Remote/field-based role; preferred to live in specified geography. Bilingual proficiency in certain areas is preferred. In addition, this position is eligible for participation in Pfizer’s Global Performance Plan with a bonus target of 20.0% of the base salary and eligibility to participate in our share based long term incentive program.
