Community Nurse Liaison LPN

Description American Senior Communities is now hiring a Community Nurse Liaison (LPN) Hours: Saturday and Sunday 8a - 5p Remote position but must live within reasonable driving distance to Indianapolis The Clinical Nurse Liaison provides patient evaluations, while collaborating with hospital personnel to determine patient’s clinical needs and appropriateness for admission to skilled nursing facility. This position works closely with our communities and healthcare partners. This position will also be checking benefits and payor sources.
