Job Summary The Community Outreach Specialist plays a critical role in improving patient outcomes by delivering outreach, engagement, and care coordination support focused on closing preventive and chronic disease care gaps. The primary responsibility is to execute standardized outreach workflows at a high volume, while identifying barriers, documenting outcomes, and providing feedback to continuously improve outreach strategies. By working in collaboratively with participating health centers, the Community Outreach Specialist supports the implementation of outreach strategies, patient engagement efforts, and resource coordination that help improve performance in breast cancer screening, colorectal cancer screening, blood pressure control, and diabetes management while contributing to reduced avoidable utilization and enhanced patient experience. While the position is primarily remote, occasional travel to participating health centers will be required to support workflow implementation, relationship development, and continuous quality improvement efforts across the network. What You’ll Do Conduct high-volume outreach to engage patients in preventive care and chronic disease management programs. Build trusted relationships through compassionate, patient-centered communication and education. Help patients overcome barriers to care, including transportation, scheduling, language, health literacy, and financial challenges. Support patients in completing recommended screenings and follow-up care, including breast and colorectal cancer screenings. Provide education and follow-up support for patients managing chronic conditions such as hypertension and diabetes. Connect patients with community resources and coordinate referrals, transportation, and other non-clinical support services. Accurately document patient interactions, outreach efforts, referrals, and outcomes in designated systems. Collaborate with care teams and community partners to improve patient engagement, access to care, and health outcomes. Maintain compliance with HIPAA, organizational policies, quality standards, and outreach protocols. What We’re Looking For High school diploma or GED required; associate degree or relevant certification preferred. Basic understanding of medical terminology Experience working with diverse, underserved, or community-based populations. Experience conducting in-home visits and community outreach is preferred. While this role is currently performed remotely, future program expansion may include community- or home-based outreach activities. Valid driver’s license and reliable transportation, as required for home visits. Experience in performing non-invasive health screenings (e.g., blood pressure (BP) measurement) and point-of-care testing under approved standing orders and protocols is preferred but not currently required for this position. Basic proficiency with technology, including EHRs, mobile documentation tools, or care management platforms. Preferred Qualifications: Bilingual or multilingual skills reflective of the communities served strongly preferred. Prior experience in community health, outreach, care coordination, or public health programs. Colorado Community Health Worker certification through the Colorado Department of Public Health and Environment (CDPHE) is preferred, or ability to obtain certification within a defined timeframe, as required by state or program standards. Experience performing basic clinical tasks such as BP measurement or point-of-care testing. Familiarity with social drivers of health and community-based resources.
