SUMMARY The Community Supports Manager will oversee a team of housing case managers, coordinate, and supervise programs. WHERE YOU’LL WORK This position is hybrid. Work from home and go into the field as needed. Travel is required to meet with staff and members within designated areas or based on business needs. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:00 am to 5:00 pm. WHAT YOU’LL DO Oversee CS staff schedules and duties, i.e., reviews data entry/documentations to ensure staff are properly documenting member encounters in a timely, concise manner Orients and educates new hires by meeting with them to explain their roles, the expectations in the assumed role(s), and how to abide with all policies, processes and procedures related to their role Routinely conducts huddles and one-on-one meetings with employees to communicate goals, process changes, and other work-related information Acts as first point-of-contact for complex members’ issues or concerns and directs the employee to the appropriate resources and follows up to ensure satisfactory resolution Monitors the performance of staff, identifies training needs, and implements a performance improvement plan, if necessary Addresses employee questions and processes staff requests in a timely manner Maintains knowledge of community services and resources available to employees and ensures staff follow established protocol around child/elder abuse, members experiencing pain, safety concerns and/or mental health concerns and ensures safety plans are in place or reports situations to own direct report leader Ensures that the CS staff researches, establishes, and maintains a list of available housing opportunities in their respective region Develops partnerships with local providers and becomes knowledgeable about local community resources including transitional housing, shelters, and permanent housing Ensures that the individualized housing plan assessments are up-to-date with DHCS requirements Monitors CS staff services and ensures that CS staff communicates the plan and housing opportunities to their members to ensure effective service and that any placements are consistent with the members overall care plan Monitors CS staff services to ensure members are being assisted with their housing search, including applying for apartments, negotiating with potential landlords, filling out rental applications, including understanding tenant rights and responsibilities Ensures CS staff practices high advocacy level through assisting member's with talking to their landlord by being mediators if there is a disagreement or concern, if needed Monitors CS staff follow-up meetings with their members after housing placements to assist with any issues that may arise Ensures that CS staff utilize the Homeless Management Information System (HMIS) to add members into housing portal Ensures CS staff assist their members in completing CES assessments.
