Location: Swansea & Neath area (home, office and community based)
Contract type: Fixed Term Contract Until 31st March 2027
Hours: 35 hrs per week
Salary: £25,541 - £27,200 per annum
Driving: Full Clean UK Driving Licence and access to a vehicle. Mileage paid
Could you champion the seamless delivery of our project for individuals with dementia and their family members. Are you passionate about making a difference in the lives of in your community empowering them to navigate challenges?
We're on the lookout for a dynamic and proactive individual to lead an exciting project aimed at supporting individuals with dementia and their families throughout the region.
This unique role calls for someone with exceptional interpersonal and communication skills, a knack for motivating others, and an understanding of the diverse challenges faced by individuals with Dementia and their families.
Your mission? To reach out to Dementia individuals across Swansea and Neath, offering helpful advice, info, practical support, and personalised guidance through a holistic approach. By developing and implementing person-centred carers plans, you'll play a pivotal role in empowering them to navigate their journey with confidence and resilience. Are you up for the challenge?
