Job Summary We are seeking an experienced Epic Healthy Planet with Campaigns Consultant to support a large-scale healthcare technology initiative focused on population health management, patient engagement, and outreach optimization. This consultant will serve as a subject matter expert for Epic Healthy Planet and Campaigns, partnering with operational, clinical, and technical teams to design, implement, optimize, and support population health and patient outreach workflows. The ideal candidate has extensive experience working with Epic Healthy Planet, Campaigns, and population health initiatives, with a strong understanding of care management, preventive care programs, patient engagement strategies, and reporting capabilities within Epic. Assignment Details: Initial 6-month contract with likelihood of extensions Approximately 25% onsite as needed throughout the engagement Opportunity to support a large-scale healthcare transformation initiative Key Responsibilities Serve as the primary Epic Healthy Planet and Campaigns subject matter expert. Gather business, clinical, and operational requirements related to population health, care management, patient outreach, and quality initiatives. Configure, optimize, and support Epic Healthy Planet and Campaigns functionality. Design and implement patient outreach strategies using Campaigns and related Epic tools. Collaborate with clinical, operational, analytics, and leadership teams to support population health initiatives and patient engagement programs. Build, maintain, and optimize campaigns, registries, worklists, dashboards, and reporting solutions. Support quality, preventative care, risk stratification, chronic disease management, and care gap closure initiatives. Participate in workflow assessments and recommend system enhancements to improve operational efficiency and patient outcomes. Support testing activities, including unit testing, integrated testing, user acceptance testing, and issue resolution. Develop and maintain project documentation, build specifications, workflow diagrams, and training materials. Provide go-live, stabilization, and post-implementation optimization support. Partner with Epic application teams, reporting teams, and operational stakeholders to ensure successful project delivery. Requirements Required Qualifications Minimum of 5 years of Epic Healthy Planet experience. Experience implementing, supporting, or optimizing Epic Healthy Planet and Campaigns. Strong knowledge of population health management, patient engagement, care management, and preventative care workflows. Experience gathering requirements, performing workflow analysis, and translating business needs into Epic solutions. Experience supporting testing, validation, and issue resolution activities. Strong analytical, troubleshooting, and problem-solving skills. Excellent communication and stakeholder management abilities. Experience working in complex healthcare environments. Preferred Qualifications Epic Healthy Planet certification. Epic Campaigns experience strongly preferred. Experience supporting value-based care, quality measures, risk adjustment, or care management initiatives. Experience with registries, dashboards, reporting, and population health analytics.
