Family Coordinator - Healthcare

We are looking for a Family Coordinator to join our team full-time. This position manages newly authorized client cases from confirmed authorization through early service stabilization. This includes coordinating service readiness, supporting initial staffing alignment, and ensuring families and providers are guided through the start-of-care period. What you'll do: Serve as the primary point of contact for families during service initiation Guide families through onboarding steps, timelines, and expectations Coordinate assessments, provider assignment, and service start Ensure services are scheduled, and providers are ready to begin Track early service delivery and resolve issues impacting consistency Partner with internal teams to maintain timelines and service readiness Coordinate the transition of stable cases to Case Management What you need: Advanced English Knowledge. Customer Service skills needed to handle the Caregivers/Patients/Insurance interaction. Organization skills needed to handle all the important documentation that is required in this position. Strong organizational and coordination ski ls in a fast-paced environment Clear, professional communication across multiple stakeholders Ability to manage timelines, priorities, and folow-through Problem-solving mindset with focus on execution Attention to detail and accountability for outcomes Proactivity and creativeness its needed for this position, someone proactive as opposed to reactive.
