About the Role As a Field Nurse Case Manager , you will play a critical role in supporting patients throughout their treatment journey within a Patient Support Program (PSP). Acting as a trusted healthcare partner, you will provide education, clinical support, case management, and therapy coordination while working collaboratively with patients, healthcare providers, pharmacies, and internal reimbursement teams. This role combines patient care, care coordination, and healthcare navigation to help improve patient outcomes, facilitate access to therapy, and deliver an exceptional patient experience. The successful candidate will have a passion for patient advocacy, strong clinical knowledge, and the ability to build trusted relationships with healthcare professionals across Ontario. What You'll Do: Deliver patient-centric case management services within established program protocols. Support patients throughout their treatment journey, including onboarding, education, adherence support, and ongoing follow-up. Provide disease-state, therapy, and medication education to patients and healthcare providers. Collaborate with physicians, clinics, pharmacies, and internal teams to coordinate patient care and access to therapy. Work closely with Reimbursement Specialists to support funding, prior authorizations, and patient access solutions. Conduct telephone-based and virtual patient follow-up activities, including adherence and monitoring calls. Monitor treatment progress, therapy interruptions, discontinuations, and patient outcomes in accordance with program requirements. Document patient interactions accurately and maintain complete, timely records within program systems. Identify and report adverse events (AEs) and product complaints in accordance with program, regulatory, and company requirements. Participate in ongoing product, therapy, and disease-state training. Attend occasional educational events, conferences, and clinic meetings to support patient and provider engagement. Build and maintain strong relationships with healthcare providers and clinic staff to ensure a seamless patient experience. Maintain the highest standards of patient confidentiality and privacy compliance. Basic Requirements: Diploma or Degree in Nursing. Current unrestricted registration as an RN, RPN, or LPN/LVN equivalent licensed in Ontario and in good standing with the applicable regulatory body. Experience providing direct patient care, patient education, patient support, healthcare coordination, or case management. Strong verbal and written communication skills in English. Proficiency with Microsoft Office applications, including Outlook, Word, and Excel. Ability to effectively manage multiple priorities in a fast-paced environment. Must reside in Ontario and be legally authorized to work in Canada. Preferred Skills/Experience Experience working within a Patient Support Program (PSP) , specialty pharmacy, reimbursement, infusion clinic, pharmaceutical, or healthcare setting. Bilingualism (English/French). Active nursing licensure in additional Canadian provinces. Experience supporting specialty therapies, chronic disease management, biologics, oncology, neurology, immunology, or rare disease programs. Knowledge of reimbursement processes, funding navigation, and patient access programs. Experience with case management platforms, CRM systems, or electronic health records (EHR/EMR). Strong problem-solving, relationship-building, and patient advocacy skills. Travel & Working Conditions Hybrid field-based role with occasional travel throughout Ontario for clinic visits, patient education activities, conferences, and stakeholder meetings. Valid driver's license and access to reliable transportation may be required. Primarily remote/home-office based when not travelling. Frequent computer and telephone use required.
