Baptist Health is hiring a Follow Up Care Coordinator to join the BPE Physician Integration team in Jacksonville, FL. This is a full-time, hybrid opportunity located at Southbank. The Follow Up Care Coordinator supports patients following encounters at Baptist Free Standing Emergency Departments and CareSpot locations by assisting with follow-up care needs, connecting patients with primary care providers, and coordinating access to healthcare services. This role serves as a liaison between patients, clinics, providers, and community resources while delivering exceptional customer service and personalized healthcare coordination. Follow Up Care Coordinator Job Responsibilities: Provide assistance to patients following encounters at Baptist Free Standing Emergency Departments and CareSpot locations Support patients with follow-up care needs and help connect them with family medicine physicians Maintain and utilize the department’s customer relationship platform, SalesForce Build and maintain relationships with Baptist Health, Baptist Primary Care, CareSpot, leadership, and clinicians Inform and educate stakeholders about resources, services, providers, and facilities Assist consumers with unique healthcare interests and needs Support key initiatives including the New Mover's Program, Corporate Health (PATH), and the Women's Health Strategy (4HER) program Schedule patient appointments directly within EPIC for Baptist Primary Care clinics Serve as a liaison between patients and clinic offices on strategic service line initiatives Support patient access, healthcare coordination, and preventive care efforts Deliver a concierge-level, personalized approach to patient care coordination Meet departmental quality and performance metrics related to customer experience, including call availability, abandon rate, and average handling time Follow Up Care Coordinator Experience: 1-2 years Database Maintenance Experience (required) 1-2 years Customer Service Experience (required) Follow Up Care Coordinator Education & Credential Requirements: High School Diploma (required) Bachelor of Arts/Science (BA/BS) (preferred) Schedule: Monday - Friday, 8:00 AM - 5:00 PM One-hour lunch break Eligible to work from home every Friday after successful completion of the 90-day probationary period If you are interested in this Follow Up Care Coordinator opportunity with Baptist Health , please apply today!
