As a Health Transformation Clinical Pharmacist, you’ll work directly with providers, care teams, and patients to improve medication access, optimize therapy outcomes, and support innovative care delivery models across multiple specialty service lines. This role offers the opportunity to blend direct patient care, care coordination, and healthcare innovation while supporting programs that expand access and improve the patient experience across rural healthcare communities. Job Summary The Health Transformation Clinical Pharmacist provides clinical pharmacy support focused on ambulatory care, specialty services, medication access, and HealthConnect One (HCO) patient care initiatives. This position collaborates closely with physicians, nurses, care coordinators, and interdisciplinary healthcare teams to optimize medication therapy management, improve patient access to care, and support innovative healthcare transformation initiatives. The role supports current and future specialty-focused programs including oncology, cardiology, rheumatology, dermatology, behavioral health, transitional care, and other ambulatory service lines. This position also serves as a patient-facing pharmacist supporting HealthConnect One pharmacy services and medication access programs. Key Responsibilities Clinical Pharmacy & Patient Care Evaluate medication therapies for safety, effectiveness, appropriateness, and cost efficiency. Provide ambulatory care and specialty pharmacy support across multiple clinical service lines. Assist with management of complex medication regimens for high-acuity and specialty patient populations. Support pharmacist-directed patient care initiatives including chronic disease management and medication optimization. Utilize telehealth, mobile integrated care, and other care delivery technologies to improve patient access and outcomes. Care Coordination & Medication Access Collaborate with internal and external healthcare teams to coordinate medication therapy and care transitions. Support prior authorizations, specialty medication onboarding, and therapeutic initiation scheduling. Improve patient access through payer coordination, formulary management, and medication assistance initiatives. Assist with HealthConnect One (HCO) patient care services and pharmacy access programs. Service Development & Operational Support Assist with development and implementation of ambulatory and specialty pharmacy initiatives. Support healthcare transformation projects focused on patient experience and population health management. Analyze health economic and service-level data to identify operational improvements and expanded patient access opportunities. Assist with vendor, payer, and service agreement evaluations and implementation activities. Support pharmacy operations, compliance initiatives, and healthcare logistics planning. Education & Collaboration Educate patients, providers, nursing teams, and interdisciplinary staff on medication therapies and best practices. Collaborate with physicians, care coordinators, and leadership teams to improve quality outcomes and care delivery. Contribute to innovative clinical and operational initiatives across the Health Transformation Department. Qualifications Required Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited program. Current licensure as a Pharmacist through the Arkansas State Board of Pharmacy. Experience in pharmacy, healthcare, health services, or related clinical environments. Strong communication and interdisciplinary collaboration skills. Ability to read, write, speak, and understand English. Preferred MBA, MHA, or related healthcare administration experience. Minimum of 2 years of hospital pharmacy experience. Minimum of 2 years of ambulatory care, transitions-of-care, or clinical pharmacy services administration experience. Experience with 340B program operations within a health system. Experience with healthcare service-line creation, implementation, or operational development. Knowledge of pharmacy purchasing and pharmacy revenue cycle integrity. Apexus-Certified 340B Expert (ACE) certification. Board Certification through the Board of Pharmacy Specialties (BPS).
