Provides customer service to patients, families, providers, legal entities, and third-party requestors while ensuring accuracy, confidentiality, and timeliness. Utilizes electronic health records and related systems to retrieve, review, and release information. Supports quality, regulatory, and operational standards through accurate documentation and efficient workflow management. Essential Functions • Receives, reviews, and processes requests for medical records from patients, providers, attorneys, insurers, law enforcement, and other authorized parties. • Screens requests for completeness and compliance with HIPAA, state, federal, and enterprise policies before releasing information. • Retrieves and prepares records from electronic systems, paper files, microfilm, and off-site storage. • Certifies records, processes amendments, and ensures proper documentation of all releases. • Responds to subpoenas, court orders, and other legal requests in collaboration with Legal, Risk, and Privacy teams. • Provides customer service by answering calls, responding to inquiries, and assisting walk-in visitors with record requests. • Releases information through approved methods including fax, mail, portal, electronic media, or secure transfer. • Invoices requestors, reconciles payments, and manages outstanding balances in compliance with contracts and policies. • Documents all requests, releases, and actions in EPIC ROI and other health information systems. • Maintains productivity, accuracy, and quality standards while safeguarding confidentiality. • Performs other duties as assigned. Education Requirement: High school diploma or equivalent experience Experience Requirement: 2-3 years of experience An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week.
