Primary Location: Remote This position is designated as Remote and primarily performs work from an approved remote work location. Employees may be required to attend in-person meetings, trainings, organizational events, retreats, or program visits based on business needs. Job Summary: We currently have an opening for an Home Health Agency Director (Temporary Assignment)). The nurse manager will oversee an agency that includes RNs, LVNs, PTs, and OTs. The Home Health Agency Director will be responsible for quality, compliance, strategy, auditing, and overseeing agency personnel. They will review documentation for quality and compliance, and support all agency activities related to client safety, satisfaction, and clinical outcomes. TLC often services clients who reside in unstable and/or non-traditional housing environments, such as shelters or unsheltered areas. About the role: Essential Duties and Responsibilities: Agency Management: Oversee daily operations, including personnel management, budgeting, and compliance with regulations. Supporting activities related to licensure, regulations, and reporting Policy & Procedure: Develop, implement, and maintain policies and procedures related to patient care and agency operations. Financial Management: Participate in budget development and monitoring, ensuring efficient use of resources. Compliance: Ensure adherence to all federal, state, and local regulations, as well as accreditation standards like those of the Joint Commission. Staff Management: Partner with the HH Nurse manager to orient and evaluate clinical and administrative staff. Strategic Planning: Contribute to the development of agency goals and objectives and participate in long-term planning. Clinical Standards: Ensure care is delivered according to established clinical standards and best practices. Applying infection control principles Quality Assurance: Monitor patient outcomes, participate in quality improvement initiatives, and ensure high-quality care. Assessing agency activities for quality and compliance Conducting chart audits and supervisory visits Interdisciplinary Team Management: Lead and participate in team meetings, fostering communication among healthcare professionals. Plan of Care Development: Auditing patient care plans Staff Competency: Ensure clinical staff are qualified and competent, providing ongoing support and supervision. Regulatory Compliance: Stay current with healthcare regulations and ensure agency compliance. Additional duties include: Accepting clinical assignments aligned with skills and qualifications Completing documentation promptly Participating in continuing education and in-service programs Communicating effectively with families and staff Promoting company philosophy and policies Demonstrating professionalism and ongoing growth Minimum Qualifications: Licensure/certifications: Valid California RN or LVN license, CA Driver’s License, current CPR proof. Skills: Nursing skills according to accepted standards, strong interpersonal and leadership skills, knowledge of home health needs, and compliance requirements. Transportation: Reliable vehicle and valid auto insurance. Experience: Minimum one year of home health nursing required. Preferred Qualifications: Clinical Expertise - A strong clinical background and an HHA RN with experience in clinical care, administration, quality, and compliance. Management and Leadership Skills - Proven ability to manage and lead a team, making sound clinical and administrative decisions. Communication and Interpersonal Skills - Excellent skills for effective collaboration with staff, patients, and families. Problem-Solving and Critical Thinking - Ability to identify and resolve issues related to patient care and operations. Knowledge of Home Health Regulations - Deep understanding of federal, state, and local home health laws. Financial Management Skills - Knowledge of budgeting, financial reporting, and resource allocation. Physical Requirements: This position is primarily sedentary in nature. Essential physical activities may include: Prolonged periods of sitting. Frequent computer and keyboard use. Frequent use of telephones and virtual meeting platforms. Occasional standing and walking. Occasional reaching, bending, or carrying office materials. Ability to lift, carry, push, or pull up to 15 pounds with or without reasonable accommodation. Work Environment: The position is primarily remote. Administrator/DPCS will be onsite at least quarterly to conduct in-person visit audits, in-person training, and will be present onsite for audits such as CDPH and JCAHO. Work Environment: Cardea Health program locations operate within supportive housing, interim housing, medical respite, and community-based environments serving individuals experiencing homelessness and other vulnerable populations.
