Implementation Assistant – Case Management

Job Description This is a remote position. We are seeking an organized and proactive Implementation Assistant to support the onboarding of new clients and ensure a smooth transition for both clients and the Guided Care Case Management team. This role provides administrative support throughout the implementation process, maintains user access to the Guided Care portal, responds to client requests, and coordinates with multiple case management teams. The ideal candidate is self-motivated, detail-oriented, and comfortable working independently in a fast-paced remote environment. Responsibilities Support the Manager of Implementation and Data Integrity and Implementation Specialists with administrative tasks. Enter and maintain facility contacts, EMR access notes, and other information in the Guided Care portal. Manage external-user access and security within the portal. Process ongoing portal-access requests. Respond to client emails and requests by the end of the business day. Coordinate with clients and case management teams to support successful onboarding. Maintain accurate, confidential client and employee information. Evaluate personal performance and identify opportunities for improvement. Complete additional duties as assigned. Requirements Strong written and verbal communication skills. Excellent organization and time-management abilities. Ability to work independently and collaborate with a remote team. Comfortable working in a fast-paced environment with frequent interruptions. Ability to manage multiple priorities and meet deadlines. Strong discretion when handling confidential information. Proficiency with Microsoft Office and ATS platforms. Ability to quickly learn new systems and software. Experience maintaining positive client and team relationships
