About Conduit Health Conduit Health is transforming one of the hardest, most outdated corners of post-acute care — getting essential medical equipment and supplies into patients’ homes. We’re the first vertically integrated, AI-powered platform to unite ordering, telehealth, prescriptions, insurance, and fulfillment into one seamless experience. In seconds, case managers and providers can say “yes” to patients who would otherwise wait weeks — while we handle every step behind the scenes. The Role We’re looking for an Implementation Manager to optimize state launches, payor implementations, partnerships, or high-impact projects across the company. You’ll work closely with multiple teams to plug in where the team needs most. This role offers a unique opportunity to shape operations and expansion in a high-growth startup environment, working hands-on with industry leaders and driving impact across our patient population. The Implementation Manager leads new implementations - state expansions, payor launches, and cross-functional initiatives - from buy-in through execution, reporting to the Head of Operations. Sitting on the DME Operations team, they own the project management of these launches and act as the connective tissue across Clinical Ops, Support, Payor Development, and Product, actively gathering deliverables to keep commitments on track and things moving. The role fills the gap between strategy and delivery: someone accountable for making sure new initiatives actually land, rather than stalling in the handoffs between teams. Who You Are Organization is in your DNA. You bring clarity to chaos, simplify complex details, and surface information for who needs it most. Execution-Focused & Detail-Oriented: You know what is needed to take a project from 0 to 1, and you thrive in organizing both the small and big details. Collaborative Leader: You get buy-in across teams and levels across the organization. Project Management: You keep team members and to do lists on track to get us where we need to go. Tech-Savvy & Efficiency-Focused: You use systems, tools, and metrics to drive continuous improvement. Examples of What You'll Do Own new state expansions - from post-diligence handoff through go-live - building the launch plan, timeline, and punch list, and driving every workstream to its date. Lead payor implementations and launches, coordinating the eligibility, credentialing, billing, and fulfillment readiness that turns a signed contract into live patients. Act as the connective tissue across M&A, Clinical Ops, Support, Payor Development, and Product - chasing commitments, unblocking handoffs, and keeping launches moving when they would otherwise stall. Build and own the source-of-truth launch and expansion trackers so everyone knows what is done, what is next, and who owns it. Run kick-offs, surface risks early, and course-correct before a slipped dependency becomes a missed launch. Potentially own the network contracting admin the land-and-expand model runs on, standardizing work currently split across the team. Report against launch KPIs - on-time launches, states executed, payors launched - and turn what you learn into repeatable playbooks. What You'll Bring Organization and structure-building are your superpower - you turn ambiguity into clear plans, trackers, and detailed punch lists (eg via Miro, Figma, Excel, Asana, Jira). Proven project management: you can point to an initiative you took from 0 to 1 and explain exactly what moved it (even if you did not do every piece yourself). Stakeholder management and a track record of getting buy-in across teams and levels. Expert-level, clear communication tailored to different stakeholders - no AI slop, no jargon dumps. High attention to detail and the ability to hold many moving parts at once without dropping threads. A passion for using systems, tools, and metrics to improve processes and the patient experience. Bonus points if you have: Management consulting or a similar structured problem-solving background - this role is a natural landing spot for someone moving from consulting to in-house. Healthcare, DME, or health-tech experience. A real plus, but not required - we will teach you the domain. Experience in a high-growth startup or scaling environment. Exposure to M&A integration or post-acquisition operations. Values Excellence, Not Perfection – We set a high bar for our work and impact, but we don’t get lost in endless polish for polish’s sake. We focus on results that matter. Urgency, Not Chaos – We move fast because speed drives our advantage. We pair urgency with clarity to avoid noise and burnout. Systems, Not One-Offs – Every solution should eliminate the root cause so the problem never returns. Wins are good; scalable systems are better. Committed, Not Just Here – We show up engaged, proactive, and ready to go above and beyond. It’s not about working 24/7 — it’s about caring deeply and leaning into our shared mission. The outcomes you drive are what matter most. Crush Goals, Not Souls – We take our work seriously, but not ourselves. Be yourself, bring positivity, have fun, and laugh often. Compensation & Benefits Competitive salary with performance-based incentives. Flexible working environment (3 in-office days per week in New York). Unlimited PTO + 9 company holidays. Direct mentorship and growth opportunities with senior leadership. Equal Opportunity Conduit Health is an equal-opportunity employer. We celebrate diversity and are committed to building an inclusive environment for all employees. Location: New York City | Type: Full time | Hybrid | Department: Operations | Salary: 100k-140k
