Job Summary : K-day PACE is committed to helping older adults remain safely and independently in their homes and communities through comprehensive, coordinated, and culturally attuned care. Our Caregivers are an important part of this mission, providing compassionate in-home personal care and support to K-day participants. The Caregiver provides personal care and assistance with activities of daily living in accordance with each participant’s individualized plan of care, authorized Home Care services, assigned care instructions, and K-day PACE policies and procedures. The Caregiver promotes participant safety, dignity, independence, and quality of life and serves as an important source of observation and communication for the K-day care team. Caregivers promptly report relevant changes in a participant’s condition, functioning, behavior, environment, or care needs to the Home Care Coordinator and/or appropriate clinical team. The Caregiver provides only services for which they have been appropriately trained, assigned, and determined competent and does not independently modify a participant’s plan of care, authorized hours, services, or clinical treatment. Qualifications and Requirements : Education/Training/Certifications: High school diploma or equivalent required. Current CPR, AED, and First Aid Certification required Current driver’s license and proof of auto insurance Must successfully complete K-day PACE orientation, caregiver training, and position-specific competency validation before independently providing participant care. Must meet all applicable background, exclusion-screening, health-clearance, and other employment requirements. Experience: Must have a year of experience with a frail or elderly population Prior caregiving, personal care, IHSS, home care, senior care, CNA/HHA, community-based care, or long-term care experience preferred. Two years of caregiving or home/community-based elder-care experience preferred. Medical Clearance: Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Duties and Responsibilities : Provide hands-on personal care and assistance with Activities of Daily Living (ADLs), including but not limited to bathing, grooming, dressing, toileting, eating, mobility, positioning, ambulation, and transfers. Assist participants with authorized Instrumental Activities of Daily Living (IADLs), including but not limited to participant-related laundry, light housekeeping, shopping when assigned, and light meal preparation. Provide companionship, supervision, cueing, and supportive assistance as identified in the participant’s care plan. Encourage participants to maintain the highest practical level of independence while protecting dignity, privacy, safety, and personal choice. Follow the participant’s authorized care plan, assigned task list, schedule, frequency, and duration of services. Do not independently add, discontinue, increase, decrease, or substantially change a participant’s authorized Home Care services or hours. Measure and document basic vital signs only when specifically instructed by K-day clinical staff and after appropriate training and competency validation. Assist the Rehabilitation team with participant-specific home exercise, mobility, or functional programs when included in the care plan and after appropriate instruction and competency validation. Observe participants for changes in condition, function, cognition, mobility, behavior, appetite, skin condition, safety, or ability to perform usual activities and promptly report concerns according to K-day escalation procedures. Promptly report falls, injuries, participant refusals, unsafe conditions, significant behavioral changes, or other concerns to the appropriate K-day team member. Follow K-day emergency procedures and contact emergency services when indicated. Accurately document each visit, including required clock-in/out, services provided, assigned tasks completed, tasks refused or not completed, and required participant/representative verification. Immediately notify the Home Care Scheduler regarding call-outs, lateness, participant cancellations, inability to reach a participant, schedule conflicts, or inability to complete a scheduled visit. Do not independently exchange caregiver assignments or modify scheduled visits without approval from K-day Home Care. Maintain participant confidentiality and comply with HIPAA, participant-rights requirements, professional-boundary expectations, and K-day policies. Promptly report suspected abuse, neglect, exploitation, abandonment, theft, or other participant-safety concerns according to K-day policy and applicable reporting requirements. Follow organizational policies, OSHA requirements, infection-control standards, emergency-preparedness procedures, and safe-work practices. Participate in required training, continuing education, competency evaluations, quality-improvement activities, and participant-specific training. Support and advocate for the PACE model and best practices in care for older adults in home- and community-based settings. Travel between participant homes and K-day locations as required by assigned duties.
