As a remote IKC Kidney Palliative Care Social Worker, your primary focus will be guiding patients and their families through the challenging stages of chronic kidney disease (CKD) and End Stage Kidney Disease (ESKD), including end of life. Key Responsibilities: Palliative Care Social Worker : Work directly with patients and families: Palliative Care : Provide support to patients with advanced kidney disease and their caregivers through goals of care conversations, palliative social work assessment, behavioral health interventions, and advance care planning. Education and Advocacy : Educate patients, families, and caregivers about available community resources and palliative and hospice care services. Advocate for patient needs and preferences. Team Collaboration: Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients. Liaise with dialysis clinic staff, nephrology practices, and outside providers to accomplish goal-concordant care for patients. Qualifications: Education: Master’s degree in Social Work from an accredited institution. Licensure: State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent is required. Active license in the state of California. Experience: One year of hospice or palliative care experience required. At least 6-8 years of social work experience, preferably in healthcare settings. Two years of renal experience preferred. Skills: Strong interpersonal and communication skills; ability to provide quality patient care in a remote work setting; thorough assessment skills. Knowledge: Familiarity with palliative care principles, grief counseling techniques, advance care planning processes, and behavioral health interventions. Attributes: Empathetic, self-directed, adaptable, and able to handle emotional situations with professionalism. This position will be open for a minimum of three days. The Salary Range for the role is $78,000.00 - $94,000.00 per year. If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
