Job Title LPN Central Admissions - Center for Connected Care Location Independence Facility Business Operations Center Department HC Central Intake-Center for Connected Care Job Code G1803H Shift Days Schedule 9:00am-5:30pm Job Summary Job Details **Applicants must reside in Northeast Ohio Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes. As an LPN Central Admissions, you will facilitate the timely and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for all referred patients. While you may not closely interact with patients on the job, your work will directly impact their care and make a lasting difference in their lives. This is a remote role with on-site training and scheduled work days at CC Business Operations Center located in Independence, Ohio. A caregiver in this position will work Monday through Friday from 9:00am to 5:30pm. Further requirements include weekends every 5th weekend from 8:00am to 4:30pm and 2 holidays per year from 8:00am to 4:30pm. A caregiver who excels in this role will: Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse. Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services. Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate. Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process. Serve as the CCHCS information specialist. Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited Practical Nursing Program Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) Preferred qualifications for the ideal future caregiver include: One year of clinical experience in Homecare Physical Requirements: Sitting, standing and walking for extended periods of time Manual dexterity to perform work on a PC Good visual acuity through normal or corrected vision Ability to hear typical conversation levels through normal or corrected hearing Medium Work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects Physical Demand requirements in excess of those for Light Work Personal Protective Equipment: Follow standard precautions using personal protective equipment as required The policy of Cleveland Clinic Health System and its system hospitals (Cleveland Clinic Health System) is to provide equal opportunity to all of our caregivers and applicants for employment in our drug free environment.
