Job Title: LVN Service Care Coordinator Job Description We are seeking a detail-oriented Service Care Coordinator to support a high-volume authorization team responsible for processing Long-Term Services and Supports (LTSS) requests. This role plays a critical part in ensuring timely review and processing of authorizations for durable medical equipment (DME), behavioral health services, adult day health services (DAHS), and other specialty care needs. Responsibilities Process and manage authorization requests within established turnaround times. Review and follow up on requests related to DME, mental health services, DAHS, reductions, and denials. Accurately document work completed and maintain daily production logs. Utilize multiple systems, applications, and resources simultaneously in a fast-paced environment. Communicate regularly with team members through Microsoft Teams and other virtual collaboration tools. Follow departmental work guides, policies, and quality standards. Support additional authorization functions through cross-training and team collaboration. Qualifications Bachelor's degree in Social Work, Health Services, Behavioral Science, or a related field (LVN/LPN) Minimum 2 years of experience in a managed care environment or working with individuals who have disabilities, chronic conditions, or other vulnerable populations. Active LPN, LVN, or LCSW license (highly preferred) Experience with utilization management, authorizations, care coordination, or managed care operations. Experience supporting LTSS, Medicaid, or complex care populations. Strong organizational, multitasking, and time management skills. Job Type & Location This is a Contract position based out of San Antonio, TX. Pay and Benefits The pay range for this position is $20.00 - $23.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
