Mental Health Therapist (LCSW, LPC, LSW, LAC) – Bilingual Haitian Creole/English

Mindify Wellness and Care is seeking a compassionate, skilled, and client-centered Mental Health Therapist to provide individual therapy services to clients with a variety of mental health diagnoses. Working as part of a collaborative behavioral health team, the therapist will support clients in achieving their treatment goals through evidence-based interventions, care coordination, advocacy, and ongoing support. This position provides the opportunity to deliver care through a hybrid model , combining telehealth services with in-person clinical care at adult day care centers in the Asbury Park, New Jersey area. Essential Duties and Responsibilities Provide individual therapy to clients with a variety of mental health needs, including depression, anxiety, trauma, substance use disorders, and other behavioral health concerns. Develop and implement individualized treatment plans tailored to each client's needs, strengths, and goals. Conduct comprehensive clinical assessments and evaluate client progress throughout treatment. Coordinate care with other providers, including psychiatrists, psychiatric nurse practitioners, psychologists, primary care providers, and other healthcare professionals. Advocate for clients and assist with identifying appropriate community resources and support services. Provide education, guidance, and emotional support to clients and their families. Maintain accurate, timely, and compliant clinical documentation. Participate in multidisciplinary team meetings and treatment planning discussions. Engage in ongoing professional development and training activities. Deliver services through a hybrid care model, conducting sessions both virtually (telehealth/video) and onsite as needed. Ability to seamlessly transition between service modalities is required. Qualifications and Requirements Master's degree in Social Work, Counseling, or a related behavioral health field from an accredited program. Active New Jersey licensure as an LCSW, LSW, LAC, or equivalent behavioral health license required. Minimum of two (2) years of experience providing mental health services preferred. Bilingual in Haitian Creole and English required. Strong clinical assessment, therapeutic, and treatment planning skills. Excellent communication and interpersonal skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Commitment to delivering culturally responsive, equitable, and client-centered care. Work Location Hybrid position serving clients in the Asbury Park, New Jersey area. Combination of remote telehealth and in-person clinical services. Compensation $55,000–$75,000 annually , depending on licensure, experience, and qualifications.
