Midwife - Healthy Start Enhanced San Bernardino (HSSB) program

Salary Range: $41.00 To $45.00 Hourly Global Communities is seeking a Midwife to join the Healthy Start Enhanced San Bernardino Program. This role is 30 hours per week, includes benefits, and is based remotely within San Bernardino County. Under the direction of the Program Director, the Midwife will provide culturally responsive, participant-centered prenatal, postpartum, and lactation support to individuals enrolled in the Healthy Start Enhanced San Bernardino (HSSB) program. The Midwife will provide supplemental clinical support to enrolled Healthy Start clients while also facilitating two weekly perinatal education and support groups throughout San Bernardino County. The Midwife will work collaboratively with HSSB Care Coordinators, program staff, healthcare providers, and community partners to support healthy pregnancies, births, postpartum experiences, breastfeeding, and infant well-being. The Midwife will assist with the early identification of maternal and infant health concerns, provide education and support, facilitate appropriate referrals and warm hand-offs, and promote participant connections to healthcare and community resources. In addition to direct participant services, the Midwife will coordinate the planning and implementation of two weekly HSSB groups. This includes managing group schedules, dates, times, locations, educational themes, activities, and participant engagement strategies. The Midwife will provide services within the scope of their professional licensure and applicable laws, regulations, and Global Communities policies and procedures.  PRIMARY RESPONSIBILITIES: Group Education & Management Plan, coordinate, and facilitate two weekly Healthy Start perinatal education and support groups throughout San Bernardino County. Develop group schedules, locations, educational themes, activities, and participant engagement strategies. Create and deliver culturally responsive, evidence-based educational content addressing maternal, infant, and family health. Foster welcoming, inclusive group environments that encourage peer support, learning, and community connection. Manage participant recruitment, retention, attendance tracking, and follow-up activities. Coordinate guest speakers, community resources, and special group events as appropriate. Collect participant feedback and implement improvements to strengthen group participation and impact. Supplemental Prenatal, Postpartum & Lactation Support Deliver individualized, supplemental clinical support through home visits, telehealth visits, community-based visits, and other approved settings. This care is intended to be supplemental care for enrolled Healthy Start clients only and cover prenatal, postpartum, newborn care, and lactation support to Healthy Start participants. Educate participants on pregnancy, childbirth, postpartum recovery, breastfeeding, infant feeding, safe sleep, newborn care, and infant development. Support participants in developing birth plans, postpartum care plans, and infant feeding goals. Promote participant self-advocacy and informed decision-making in healthcare settings. Coordinate with Care Coordinators and healthcare providers to address participant needs and support continuity of care. Facilitate referrals and warm hand-offs to medical, behavioral health, lactation, and community-based services. Outreach, Community Partnership & Staff Training Build and maintain collaborative relationships with healthcare providers, hospitals, birth centers, lactation professionals, and community organizations. Represent Healthy Start at community meetings, outreach events, and partner activities. Support outreach efforts to increase awareness of Healthy Start services and strengthen referral pathways. Serve as a maternal and infant health subject matter expert for program staff and community partners. Provide training, coaching, and consultation to Healthy Start staff and partners on topics including pregnancy, postpartum health, breastfeeding, newborn care, and urgent maternal warning signs. Support program quality improvement efforts by sharing emerging evidence and best practices in perinatal care and lactation support. Documentation, Compliance & Program Reporting Complete timely, accurate, and thorough documentation of participant services, assessments, referrals, and follow-up activities in required data systems. Maintain records in accordance with Healthy Start requirements, professional standards, HIPAA, and Global Communities policies and procedures. Track participant outcomes, group participation, referrals, and service delivery activities. Assist with data collection, reporting, and quality assurance activities. Maintain current licensure, certifications, and continuing education requirements. Required Qualifications Licensed Midwife (LM) in the State of California in good standing or Certified Nurse Midwife (CNM) licensed to practice in California. Minimum three (3) years of experience providing prenatal, postpartum, newborn, and/or lactation care. Demonstrated experience working with pregnant and parenting families in community-based settings. Experience supporting populations disproportionately impacted by adverse maternal and infant health outcomes, including Black families, low-income families, immigrant and refugee communities, and other underserved populations. Experience facilitating educational groups, workshops, support groups, or prenatal education programs. Strong interpersonal, communication, and relationship-building skills. Ability to work collaboratively with multidisciplinary teams, healthcare providers, and community partners. Success in this role requires strong relationship management skills, effective communication, and the ability to address disagreements constructively while preserving trust and collaboration. Ability to maintain accurate documentation and use electronic data systems. Commitment to health equity, trauma-informed care, cultural humility, and participant-centered service delivery. Participation in continuing education activities and maintenance of all required licenses and certifications. Valid California driver's license, reliable transportation, and ability to travel throughout San Bernardino County. Ability to pass required background screenings and comply with organizational policies. Certifications and Professional Requirements Current Basic Life Support (BLS) certification. Neonatal Resuscitation Program (NRP) certification preferred or required within a specified timeframe. Current professional liability coverage, if required by licensure type. For Licensed Midwives, active participation in organized professional peer review is strongly preferred. Preferred Qualifications International Board Certified Lactation Consultant (IBCLC) credential or advanced lactation training. Experience working within Healthy Start, home visiting, public health, maternal-child health, federally funded programs, or community health programs. Experience conducting home visits and/or group prenatal care. Bilingual in English and Spanish. Lived experience or demonstrated cultural connection with communities served by Healthy Start San Bernardino. Existing relationships with maternal and child health providers and community resources in San Bernardino County.
