RESPONSIBILITIES
Patient Care.
Provide primarily remote care to palliative care patients, responding to triage needs and more urgent concerns through video visits.
Patient Satisfaction.
Communicate with patients and families with empathy and understanding, providing education, support, and guidance related to their care.
Care Coordination.
Collaborate with the palliative care team to assess patient needs, provide appropriate recommendations, and coordinate care.
In-Person Coverage.
Complete required in-person training and provide occasional in-person coverage for vacation and sick-call needs.

NP - Nurse Practitioner Certification - New York State Education Department (NYSED), RN - Registered Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
