Assistant Practitioner | 10857 | fixed term contract/secondment opportunity for nine months | 37 hours per week | £33,119 to £34,811 per annum | Grade G | Norwich We have an exciting opportunity for someone who has experience of working in health and/or care roles and an interest in Occupational Therapy to join the Southern Locality team as an Occupational Therapy Assistant Practitioner. This post has been created to support our transformation work with Adult Social Services to move to more preventative, relational based input with our residents and communities. In this role, you will work on several occupational therapy related tasks to enable the existing team to develop new ways of working. We support flexible and hybrid working, with a minimum expectation of two days per week spent in a community or office setting, helping you stay connected with colleagues, communities and the people we support. You will join a supportive and friendly team working with people with complex needs living with disability and/or long-term conditions in the community. The role includes carrying out less complex assessments of people’s functional ability and presenting difficulties, using a strengths-based and facilitative approach to meeting their needs. Assessments may be face to face or virtual, utilising technology when available and appropriate or visiting people in their home environment. It could also lead to sign posting and liaising with other agencies and/or providing low level equipment to maximise independence in everyday activities. You will be required to keep up to date records on a client record system and may support our regular duty, triaging new referrals as they come in. You will receive regular supervision and case load management support by the Practice Consultant. Training will be provided in equipment provision and minor housing adaptations as required. You will need excellent communication, record keeping and keyboard skills. Some knowledge of The Care Act and its provision for ‘Prevent, Reduce, Delay’ before the need for formal services would be an advantage. To build meaningful relationships with the people we support, this role requires regular in-person work in both the office and community. South Norfolk is a large, diverse geographical and socioeconomic area. You will need to have a driving licence and regular access to a car to fulfil the role.
