The Peer Recovery Coach is a full-time hybrid position in Dowagiac, MI. Position Summary: Provides peer recovery support services to patients involved in substance abuse treatment services. Uses strengths-based approaches to assist patients in achieving sustained recovery from their substance use disorder by building relationships and community support networks. Essential Functions: · Travels to patient’s residences and/or service within PHS to provide outreach and peer support. · Relates own recovery process in a positive and supportive manner through coaching, role modeling, and mentoring in order to assist patient’s in developing skills to reach their goals. · Works with patients on a self-developed recovery plan to be shared with relevant parties including Behavioral Health counselors and/or Primary Care Physician · Communicates with Behavioral Health and/or Primary Care Physician patient’s progress toward recovery plan goals. · Works with patients to navigate and use community resources in an effective manner. · Provides instruction in activities of daily living such as nutrition, meal planning, shopping, meal preparation, budgeting, home cleaning, and organization. · Completes documentation for the Behavioral Health Program and records in accordance with agency and funding source requirements. · Provides crisis intervention as needed within scope of recovery practice. Makes referrals as appropriate and notifies supervisor of crisis situations. · Plans and facilitates educational and recreational activities for patient’s and the community. · Facilitates regularly held recovery support meetings in the community · Participate in case conferences and communicate results from services through timely sharing of outcomes. · Advocate for patients for the protection of their rights. · Collaborate with outside providers, court systems, probation, DHHS, etc. · Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications and researching governmental rulings. Maintain required education credits to maintain required certifications. · Assist with developing policies and operating procedures in alignment with the Pokagon Band’s Vision, Mission, Expected Outcomes and Strategic Initiatives. Maintain operations by following policies and procedures and tribal and applicable federal, state, and local legal requirements. Non-Essential Functions: · Participate in cultural events to promote services. · Perform other related functions as assigned. Equipment: · Standard office equipment. · Systems used: Electronic Health Record – EPIC, MS Office Applications, Internet. Position Requirements: · Education: High School graduate or equivalent, preferred associate's degree in arts/sciences. · Recovery Coach Certification preferred. Must be willing to attain if not currently certified. · Lived experience as an individual who misused drugs and/or alcohol. · Must be successful in own recovery for a minimum of four years and prepared to share experience and knowledge with others. · Experience in a healthcare facility. · Must maintain patient confidence and protect operations by keeping information confidential. · Proficiency with MS Office Applications preferred. · Excellent communications and interpersonal skills required. · Experience working with Native American communities; knowledge of Native American traditional practices and values and Native American issues preferred. · Knowledge/Skills and Abilities: o Must have valid driver’s license and be able to meet the minimum insurance requirements regarding driving record in order to utilize tribal vehicles. o Must be courteous and punctual. o Must be willing to work nights. o Strong belief in family centered practice and Recovery Oriented Systems of Care. o Comfortable working independently in the community setting. o Open-mindedness about multiple paths to recovery. Indian Preference · Pokagon Band Preference Code applies. Physical Requirements: · Ability to lift and transport materials/supplies to health fairs, Pow Wows, presentations and/or other public forums. Work environment: · Flexible hours including some weekends and evenings. · Travel Required. · Services will be provided in the community, at Behavioral Health Services office, or other community setting. · Transport recoverees and their family members as needed to meet Recovery Plan Objectives.
