As a leading provider of ADHD and ASD diagnostic services, we are committed to making neurodiversity support accessible, compassionate, and life-changing. We're now looking for a warm, organised, and proactive Post Diagnostic Coordinator to become a key part of our Service Delivery team.

The Role

As our Post Diagnostic Coordinator, you'll be one of the first people our clients and families speak to after diagnosis, helping them navigate the next stage of their journey with confidence and reassurance.

Reporting directly to the Post-Diagnostic Service Delivery Manager, you'll coordinate appointments, support medication titration pathways, manage client communications, and work closely with our clinicians to ensure every client receives an outstanding experience. You'll play a vital role in ensuring Shared Care Agreements are issued promptly to GP surgeries, tracked effectively, and followed through to support a seamless transition of care. Combining empathy with exceptional organisation, you'll make sure every client feels informed, supported, and valued throughout their post-diagnostic journey.

Role Responsibilities

Client Communication

Be the friendly first point of contact for clients, families, and healthcare professionals, providing timely, compassionate support via phone and email while ensuring every client feels informed, reassured, and valued.

Appointment & Pathway Coordination

Coordinate clinician diaries, arrange titration reviews and follow-up appointments, and efficiently manage booking changes to ensure a smooth and seamless client journey.

Post-Diagnostic Care

Support clients throughout their treatment pathway by responding to queries, coordinating Shared Care Agreements with GP surgeries, monitoring their progress through the titration process, and helping ensure continuity of care between RTN and primary care providers.

Administration & Documentation

Maintain accurate client records within our systems, prepare professional correspondence, support clinicians with day-to-day administration, and help deliver an efficient, high-quality service across the wider team.

Compliance & Confidentiality

Handle sensitive information with the highest level of confidentiality, ensuring all client records, communications, and documentation comply with GDPR and organisational standards.

Continuous Improvement

Work collaboratively with colleagues to identify opportunities to improve internal processes, streamline service delivery, and enhance the experience we provide to every client.

About You

You're an organised and compassionate administrator who understands that every interaction can make a difference to someone's healthcare journey.

Experience: You have experience in customer service, administration, or communications, ideally within healthcare, mental health, or social care, and are confident managing high volumes of client communication.

Organised: You can manage multiple priorities, maintain exceptional attention to detail, and keep accurate records using digital systems.

Communicator: You have excellent written and verbal communication skills and can build rapport with clients, families, GP surgeries, and healthcare professionals alike.

Tech-Savvy: You're confident using Google Workspace, Microsoft email systems, and CRM or database software.

Professional: You understand the importance of confidentiality, GDPR, and handling sensitive information with discretion.

Passionate: An interest in ADHD, Autism, neurodiversity, or supporting people through healthcare journeys would be highly valued. Previous experience supporting clinical services, coordinating patient pathways, or managing Shared Care Agreements would be advantageous.
