At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier! Smokefree Hampshire is a smoking cessation service, which supports residents to quit smoking, vaping or nicotine use through a flexible, free 12-week programme, combining behavioural support with Nicotine Replacement Therapy (NRT). As we continue to grow and develop the service, we’re looking for a Primary Care Liaison Officer to join us. Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role, and it gives our people the chance to play to their strengths. A typical day as an Primary Care Liaison Officer might include : Building strong relationships with GP practices, Primary Care Networks (PCNs), community pharmacies and wider healthcare professionals across Hampshire. Supporting Primary Care Colleagues to increase referrals into Smokefree Hampshire services and raise awareness of available support. Acting as a key point of contact for healthcare professionals and responding to partnership enquiries. Attending practice meetings, PCN meetings and healthcare forums to promote local stop smoking referral pathways, eligibility criteria and service offerings. Supporting healthcare professionals to deliver Very Brief Advice (VBA) and smoking cessation conversations. Identifying opportunities to improve referral pathways, engagement activity and client uptake across healthcare settings. Supporting healthcare professionals to refer patients who are recorded as smoking and on the waiting well list for surgery. Working collaboratively with Public Health teams, NHS partners and community organisations to support tobacco control priorities and reduce health inequalities. Generating and sharing service performance feedback, referral and outcome data with internal and external stakeholders as required. Briefing the marketing team on the development of required service resources, campaign materials and engagement content. Securing, organising and attending outreach and engagement events in line with local priorities and current strategy. Assisting in triaging clients into our service as required. Work with colleagues across all our Thrive Tribe service to contribute to new projects and service developments – we’re always looking to share knowledge and resources! Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance. To work with us in this role, you will need to have some knowledge or experience in partnership working, stakeholder engagement, referral generation and/or healthcare engagement activity. Alongside an understanding of health inequalities and approaches to engaging priority populations into health improvement services. Bonus points if you have experience working with GP practices, Primary Care Networks (PCNs), pharmacies, local stop smoking services or wider healthcare systems! We are looking for an outstanding Primary Care Liaison Officer, working across Hampshire. If you have experience building relationships within health, wellbeing or community services, then we’d love to hear from you!
