Primary Care Provider (NP/PA/MD/DO) – Mobile Primary Care Base Salary: $150,000 + Bonus Eligibility Billet Health is seeking an experienced and entrepreneurial Primary Care Provider to join our rapidly growing Mobile Primary Care division serving the Phoenix metropolitan area. This is a full-time, Monday–Friday (8:00 AM–5:00 PM) opportunity for a provider who is passionate about delivering high-quality, patient-centered care directly in community and residential settings. About Billet Health Billet Health provides a fully integrated continuum of care that includes: Mobile Primary Care Home Health Hospice Chronic Care Management (CCM) Remote Patient Monitoring (RPM) Transitional Care Management (TCM) Advanced Wound Care Services Our mission is to improve outcomes, reduce hospitalizations, and help patients age safely in place. Practice Settings Providers will deliver care in: Assisted Living Facilities Skilled Nursing Facilities Memory Care Communities Group Homes Private Residences Retirement Communities Responsibilities Conduct comprehensive patient evaluations and physical exams Diagnose and manage acute and chronic illnesses Develop individualized treatment plans Prescribe medications and manage ongoing therapies Review diagnostics and coordinate follow-up care Perform minor procedures and wound care (preferred) Maintain timely and accurate documentation Collaborate with interdisciplinary healthcare teams Qualifications Active Arizona license as an NP, PA, MD, or DO DEA license with prescribing authority Board certification where applicable Minimum of 1–2 years of clinical experience Family Medicine experience preferred Wound care experience strongly preferred Strong clinical judgment and ability to practice independently Compensation & Benefits Base Salary: $150,000 annually Eligible for performance-based bonus opportunities Medical, dental, and vision insurance 401(k) Paid Time Off Malpractice coverage Company-provided equipment and supplies Dedicated back-office and Medical Assistant support Territory Phoenix Metro Area, including Gilbert, Chandler, Mesa, Tempe, Scottsdale, Phoenix, Glendale, Peoria, Sun City, and Surprise.
