Registered Nurse (RN) For Home Healthcare

Physicians Choice Home Health is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families. What You'll Do Provide in-home nursing visits according to the physician’s plan of care Assess patients’ health status, including condition, history, and home environment Develop and update individualized care plans based on patient needs Administer medications and treatments as prescribed Educate patients and caregivers on disease management and home care Monitor patient progress and communicate changes to the physician and care team Document visits and care in a timely manner Coordinate care with therapists, aides, and other team members Serve as case manager for assigned patients and support discharge planning Provide in-home nursing visits according to the physician’s plan of care Assess patients’ health status, including condition, history, and home environment Develop and update individualized care plans based on patient needs Administer medications and treatments as prescribed Educate patients and caregivers on disease management and home care Monitor patient progress and communicate changes to the physician and care team Document visits and care in a timely manner Coordinate care with therapists, aides, and other team members Serve as case manager for assigned patients and support discharge planning $40-125/hour, depending on visit/task type Mileage reimbursement Cellular device reimbursement Eligibility for Obran Cooperative membership and profit-sharing Full-time packages also include Medical, dental, vision, life & disability insurance, with generous employer contributions HSA/FSA options to cover healthcare expenses and dependent care with pre-tax dollars 401(k) Paid time off, sick time, and paid holidays Salary Range Disclaimer: The rates listed represent the low and high end of the range for this position.
