This position involves comprehensive patient assessments, care planning, and coordination with multidisciplinary teams to optimize patient outcomes while adhering to established clinical guidelines. The nurse will utilize telehealth technologies to monitor patient progress, provide education, and support self-management of chronic conditions, thereby reducing hospital readmissions and improving quality of life. This role requires strong clinical judgment and the ability to respond promptly to patient needs, emergencies, or changes in condition remotely. Ultimately, the Remote Care Nurse II contributes to a patient-centered care model that emphasizes accessibility, continuity, and personalized support across diverse populations within the United States. Minimum Qualifications: Current and unrestricted Registered Nurse (RN) license in the United States. Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth. Proficiency with electronic health record (EHR) systems and telehealth technology platforms. Strong communication skills to effectively engage with patients and interdisciplinary teams remotely. Ability to work independently in a remote environment while managing multiple patient cases efficiently. Preferred Qualifications: Certification in Case Management (CCM) or Telehealth Nursing (CTN) is highly desirable. Experience working in a remote or virtual care setting. Background in managing complex chronic conditions such as diabetes, heart failure, or COPD. Familiarity with healthcare regulatory requirements and quality standards related to telehealth services. Additional training or education in patient education, motivational interviewing, or behavioral health. Responsibilities: Conduct thorough remote patient assessments via phone or video conferencing to evaluate health status and identify care needs. Develop, implement, and update individualized care plans in collaboration with patients and healthcare teams. Monitor patient progress and adherence to treatment plans, providing education and guidance to promote self-care and disease management. Coordinate with physicians, specialists, and other healthcare professionals to ensure seamless care delivery and timely interventions. Document all patient interactions, clinical findings, and care activities accurately in electronic health records in compliance with regulatory standards. Respond promptly to patient inquiries and urgent health concerns, escalating issues as necessary to appropriate clinical staff. Participate in quality improvement initiatives and continuing education to maintain clinical competencies and enhance patient care services.
