We are sharing a specialised part-time consulting opportunity for experienced pharmacists with strong expertise in clinical medication review, drug interactions, dosing, contraindications, and regular use of UpToDate Lexidrug (Lexicomp) in professional practice. This role focuses on reviewing complex pharmacy and medication-management scenarios using real-world clinical judgement. Selected professionals will evaluate medication lists, patient charts, dosing decisions, contraindications, interaction findings, and clinical follow-through while applying current Lexidrug guidance and patient-specific considerations. Key Responsibilities Medication Interaction Review Evaluate complete medication lists for clinically relevant drug interactions Review interaction severity ratings, mechanisms, and implications across the patient's full medication profile Identify interactions that may be missed when reviewing only individual drug pairs Assess interaction findings within the broader clinical context Apply Lexidrug interaction guidance using professional pharmacy judgement Contraindications & Patient-Specific Appropriateness Evaluate contraindications, boxed warnings, warnings, and precautions against individual patient circumstances Distinguish between a medication being technically permitted and being clinically appropriate for a specific patient Review comorbidities, active medications, laboratory values, and other relevant patient information Identify cases where narrow screening fails to capture important clinical considerations Assess whether the appropriate monograph information has been incorporated into the decision Lexidrug Special Alerts & Monograph Review Review Lexidrug Special Alerts alongside existing monograph guidance Identify situations where newer safety information supersedes or modifies existing recommendations Evaluate whether apparent conflicts between alerts and monograph content have been appropriately reconciled Detect outdated or superseded medication guidance Apply current reference information consistently across clinical assessments Renal & Hepatic Dosing Evaluate renal and hepatic dose-adjustment decisions using current patient information Review relevant laboratory results, patient weight, renal function, and other dosing inputs Identify calculations based on stale or inappropriate clinical data Assess whether dosing calculators and adjustment logic have been applied correctly Evaluate whether resulting dose recommendations are clinically appropriate Specialised Pharmacy Review Apply strong working knowledge in at least 2–3 specialised pharmacy areas , which may include: Trissel's IV Compatibility Paediatric and weight-based dosing Beers Criteria and geriatric precautions Pregnancy and lactation considerations REMS, controlled substances, and restricted-distribution programmes Therapeutic class comparison and formulary alternatives Drug identification and patient education materials Distinguish physical or chemical IV incompatibility from physiological drug interactions Evaluate specialised medication considerations within the appropriate patient and practice context Clinical Completeness & Chart Review Assess whether medication decisions account for the patient's full clinical picture Review active medications, laboratory values, weight, comorbidities, and relevant chart information Identify questions or conclusions based on incomplete or outdated patient information Determine whether clinical assessments are scoped broadly enough to support a safe conclusion Flag omissions that materially affect medication selection or dosing Clinical Follow-Through Verify that identified medication issues are appropriately carried through into clinical action Assess whether medication orders have been updated when required Review whether prescribers or relevant care-team members have been notified Evaluate whether findings and interventions are documented appropriately in the chart or clinical note Identify cases where a correct clinical conclusion was recognised but not operationally completed Structured Evaluation & Feedback Review pharmacy and medication-management work products for clinical accuracy and completeness Identify missed interactions, dosing errors, outdated patient information, or superseded guidance Assess whether conclusions reflect appropriate use of Lexidrug resources Provide clear, evidence-based written feedback Apply consistent professional judgement across complex clinical scenarios Ideal Profile 2+ years of professional pharmacy experience Frequent use of UpToDate Lexidrug (or Lexicomp), at least weekly , in a current or recent professional role Strong familiarity with Lexidrug Interactions, Trissel's, and medication monograph sections Ability to conduct complete medication-list interaction reviews rather than isolated pairwise checks Strong understanding of contraindications, warnings, patient-specific appropriateness, and clinical medication selection Experience applying renal and hepatic dosing adjustments using current patient information Strong working knowledge in at least 2–3 specialised areas such as paediatrics, geriatrics, IV compatibility, pregnancy and lactation, REMS, therapeutic substitution, or patient education PharmD and active pharmacy licence , or equivalent verification-intensive clinical experience Experience in an inpatient or hospital setting is strongly preferred Experience in specialised settings such as ICU, oncology, paediatrics, or geriatrics is valuable Comfortable evaluating whether medication decisions were appropriately implemented through orders, documentation, and care-team communication Strong written communication and clinical reasoning skills Prior data annotation, labelling, or AI evaluation experience is advantageous Engagement Details Part-time independent contractor engagement Fully remote Flexible scheduling based on project requirements Compensation: $65–$110/hour Work focuses on clinical medication review, Lexidrug-based assessment, interaction analysis, dosing, contraindications, and medication-management scenarios Projects may be extended, shortened, or concluded based on project needs and performance Work must be completed without using confidential or proprietary information belonging to any employer, client, healthcare organisation,
