Remote Pharmacist ABOUT MEDEFER Medefer is a pioneer in virtual-first specialist healthcare. Founded by NHS consultants over a decade ago, we were delivering consultant-led virtual outpatient care well before “virtual hospitals” became a national priority. Our model has since been cited in government and NHS England material on the NHS Online vision as one of the service models informing this direction. We sit at the intersection of clinical provider and technology company. Our CQC-registered service is powered by Medefer’s own electronic health record and referral platform, connecting GPs, consultants, diagnostic providers and hospitals into a single coordinated system. This approach has won multiple national awards for elective recovery, innovation and impact, and we now serve both NHS partners and private patients across the UK. Medefer is built on three principles: innovate to solve real system problems, collaborate with clinicians and partners, and put patients first in every decision. As we scale across Integrated Care Systems and grow our private services, joining now means helping to define what modern specialist care looks like - and owning the financial strategy behind it. JOB TITLE: Clinical Pharmacist REPORTING TO: Lead Pharmacist LOCATION: Remote SALARY : Up to £50,000 per annum, pro rata Contract : 12 month fixed term contract, part time, 21 hours per week Join us in our mission to transform patient care and be part of a dynamic and innovative team. We pride ourselves on a supportive, collaborative and inclusive culture and you will have the opportunity to work alongside a team of intelligent, driven and compassionate people who care deeply about transforming the way healthcare is delivered. The role operates across multiple Integrated Care Boards (ICBs) spanning gastroenterology, rheumatology, endocrinology, and a growing range of specialties. The post-holder will work autonomously under the oversight of the Lead Pharmacist, exercising sound clinical judgement and maintaining clear communication with consultants, GPs and patients. KEY RESPONSIBILITIES: • Review consultant prescribing recommendations and undertake a full clinical pharmacy check prior to issuing a prescription, including allergies, dose, frequency, formulation, interactions, clinical appropriateness, and local ICB formulary compliance. • Issue prescriptions for NHS and private patients using remote prescribing software, in line with Independent Prescriber registration and Medefer's prescribing policies and SOPs. • Apply knowledge of local ICB formularies, Medefer operates across multiple ICBs each with distinct formulary requirements and prescribing restrictions. • Provide detailed feedback to consultants where a prescription requires amendment; utilise clinical competence to make safe changes within scope where appropriate. • Counsel patients by telephone on their medicines including indications, side effects, monitoring requirements, and when to seek further advice. • Liaise with consultants and GPs where prescriptions require clarification, amendment, or escalation. • Manage prescribing of specialist and high-risk medicines in accordance with relevant shared care arrangements and governance requirements. • Identify and escalate medicines safety concerns, interactions, and contraindications promptly. • Maintain accurate records of all clinical interventions and patient contacts. • Support the Lead Pharmacist with medicines governance activities, including audit, quality improvement and pathway development. Desirable • Experience working in a remote or virtual clinical environment. • Postgraduate certificate or diploma in pharmacy practice (PGCert or PGDip). • Familiarity with local ICB formularies and shared care protocols. • Involvement in medicines governance, SOP development or clinical audit.
