Remote RN Case Manager (Texas Residents Only) Are you an experienced Registered Nurse looking to transition from bedside care into a role focused on care coordination, member advocacy, and improving health outcomes? Required Qualifications Active, unrestricted Texas RN license in good standing Must reside in Texas 3+ years of experience in community health, public health, chronic disease management, community nursing, or case management 1+ year of experience in care coordination and disease management/patient education 3+ years coordinating care with primary care providers Outpatient case management experience within a health insurance, payer, or managed care environment Preferred Qualifications Medicaid Managed Care experience (STAR, CHIP) Marketplace or Commercial Health Plan experience Knowledge of NCQA, HEDIS, HHSC, and TDI standards CCM or other healthcare certifications Experience with Epic or other EHR systems Key Responsibilities Conduct telephonic assessments for high-risk members Develop individualized care plans and measurable health goals Coordinate services between providers, behavioral health teams, community resources, and members Educate members on chronic disease management, preventive care, medications, nutrition, and healthy lifestyle choices Support transitions of care and remove barriers to treatment Maintain accurate documentation and ensure regulatory compliance Why You'll Love This Role 100% remote schedule (Monday-Friday, 8 AM-5 PM) Opportunity to move away from bedside nursing Focus on member advocacy and relationship-building Exposure to population health and managed care initiatives Collaborative team environment with strong growth potential Make a meaningful impact on Medicaid and underserved populations Ideal Candidate The ideal candidate is an experienced RN with a managed care, health plan, or Medicaid background who enjoys helping members navigate the healthcare system, coordinating complex care needs, and improving long-term health outcomes. Job Type & Location This is a Contract to Hire position based out of Houston, TX. Pay and Benefits The pay range for this position is $36.00 - $40.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
